伴非酒精性脂肪肝的PCOS患者的临床分析

目的探讨伴非酒精性脂肪肝的多囊卵巢综合征（PCOS）患者生殖激素、血糖、血脂及体征的特征。方法将87例伴非酒精功能性脂肪肝的PCOS患者作为观察组，按时间先后顺序选取同期人院的无非酒精性脂肪肝100例PCOS患者作为对照组。比较两组患者的生殖激素、血糖、血脂及体征特征等相关指标。结果BMI、WHR、TC、TG、LDL—C、HDL—C、脂联素、FINS、HOMA—IR、T、LI-I／FSH在两组患者之间有统计学意义（P均〈0．05）；多因素Logistic分析结果显示，WHR、TG、LDL—C、HOMA—IR可能是导致PCOS患者并发NAFLD的独立危险因素，HDL—C、脂联素是PCOS患者并发NAFLD的独立保护因素。结论PCOS、NAFLD两种疾病在临床上存在相关性，PCOS伴NAFLD患者代谢异常以胰岛素抵抗、血脂异常为主要特征。对于WHR较大的PCOS患者更应该注意防范NAFLD的发生。

The clinical analysis of PCOS patients with non-alcoholic fatty liver

Objective To observe the characteristics of the clinical analysis of PCOS patients with non-alcoholic fatty liver on reproductive hormones, blood sugar, blood lipids and signs. Methods A total of 87 cases of PCOS patients with non-alcoholic fatty liver as the observation group.100 cases of PCOS which without non-alcoholic fatty liver as the control group. Comparing the reproductive hormones, blood sugar, blood lipids and signs characteristics related in- dicators between the two group. Results There were significant difference between the two groups on BMI, WHR, TC, TG, LDL-C, HDL-C, adiponectin, FINS, HOMA-IR, T and LH/FSH （t = 7.319,14.746,2.361,4.811,3.820,-3.078, 4.871,12.616,23.118,2.202, -2.524,P 〈 0.05）. Multivariate Logistic analysis showed that WHR, TG, LDL -C, HOMA-IR may be an independent risk factor for PCOS patients with NAFLD （3=0.631,0.634,0.987,3.547,P〈 0.05）, HDL-C, adiponectin may be an independent protctive factor for PCOS natients with NAFLD （β=-2.786,- 2.478,P 〈 0.05）. Conclusion The disease of PCOS and NAFLD has clinical correlation. The insulin resistance and dyslipidemia are the the main feature on this patients. The PCOS patients with greater WHR should pay attention to the prevention of the occurrence of NAFLD.
