黄芪注射液对慢性阻塞性肺疾病急性加重期患者细胞因子和肺功能的影响

目的探讨黄芪注射液对慢性阻塞性肺疾病急性加重期（AECOPD）患者血清肿瘤坏死因子-α（TNF-α）、白细胞介素8（IL-8）、白细胞介素2（IL-2）和肺功能的影响。方法将112例AECOPD患者随机分为治疗组（56例）和对照组（56例）。对照组给予常规西医治疗，治疗组在常规西医治疗基础上加用黄芪注射液40mL（加入5％的葡萄糖溶液250mL中）静脉滴注，每日1次，疗程均为14d。观察两组临床疗效，检测治疗前后血清TNF-α、IL-8、IL-2、肺功能及血气分析的变化。结果治疗组有效率为94．64％，对照组有效率为67．86％，差异有统计学意义（P〈0．05）；与治疗前比较，两组治疗后血清细胞因子TNF-α、IL-8明显下降，IL-2明显升高，且治疗组变化程度明显大于对照组（P均〈0．01）；两组肺功能及血气分析指标均有明显改善，且治疗组明显优于对照组（P均〈0．01）。结论黄芪注射液能明显降低AECOPD患者血清TNF-α、IL-8含量，升高IL-2含量，改善肺功能。提高临床疗效。

Influence of astragalus injection on serum cytokines and lung function in acute exacerbation of chronic obstructive pulmonary disease

Objective To observe the Influence of astragalus injection on tumor necrosis factor-α （TNF-α）,interleukin 8 （IL-8）,interleukin 2 （IL-2） and lung function in acute chronic obstructive pulmonary disease. Methods A total of 112 patients with AECOPD were randomLy divided into the treatment group（56 cases）and control group（56 cases）. The treatment group received 40 mL astragalus injection and 5% glucose 250 mL intravenous drip once a day at the base of conventional therapy,while the control group received conventional therapy only. The therapeutic course of both groups was 14 days. The clinical therapeutic effects of both groups were observed,the serum levels of TNF-α, IL-8,IL-2, the lung function and blood gas analysis index of both groups was measured before and after the treatment. Results The treatment group＇s efective rate was 94.64%, control group was 67.86%, diference was apparent（P 〈 0.05）. In the comparison before and after treatment,the levels of TNF-α,IL-8 in both groups were decreased obviously（all P 〈 0.01 ） ,while the levels of IL-2 were increased obviously in the two groups（all P 〈0.01 ） after the treatment,in the treatment group, the extent of change was significantly greater than that in the control group （all P 〈 0.01 ）. The lung function and blood gas analysis index of both groups were improved obviously, especially in treatment group（all P 〈 0.01 ）. Conclusion Astragalus injection may decrease obviously the serum level of TNF-α and IL-8,increase the level of IL-2,it may improve the lung function and the curative efect in the patients with AECOPD.
