摘    要:目的观察经体液免疫调节联合营养支持治疗在呼吸机相关性肺炎(VAP)患者中的疗效。方法52例发生VAP患者分为治疗组26例及对照组26例,治疗组在常规治疗的基础上。给予丙种球蛋白联合静脉营养支持,比较两组患者在营养指标、免疫功能、机械通气时间、28d存活率以及并发症的情况。结果治疗组血清白蛋白(ALB)、血清转铁蛋白(TRF)、外周血淋巴细胞计数(TLC)、免疫球蛋白IgG明显高于对照组,差异有统计学意义(P〈0.05);治疗组上机时间更短,差异有统计学意义(P〈0.05),临床有效率高于对照组,但差异无统计学意义(P〉0.05)。结论体液免疫调节与营养支持能改善呼吸机相关性肺炎免疫功能及营养状态,减少呼吸机使用时间,对改善患者的预后有一定帮助。

Efficacy of humoral immunity adjustment and nutritional support in the patients with ventilator-associated pneumonia

Abstract:Objective To observe the efficacy of humoral immune regulation and nutritional support therapy in ventilator-associated pneumonia (VAP) patients. Methods A total of 52 cases of VAP were randomly divided into a treatment group of 26 patients and the control group of 26 cases, the treatment group on the basis of conventional therapy, given the the human normal immunoglobulin and parenteral nutrition support seven days of treatment, the two groups were compared in nutrition indicators, immune function, duration of mechanical ventilation, 28-day survival and complica- tions. Results The serum albumin (ALB),~serum transferrin (TRF), peripheral blood lymphocyte count (TLC), immunoglob- ulin IgG was significantly higher than that in the control group, the difference was statistically significant (P 〈 0.05). The treatment group using ventilator time shorter, differences reached statistical significance (P 〈 0.05), the clinical ef- fective rate was higher than that in control group, but the difference did not reach statistical significance (P 〉 0.05). Conclusion The humoral immune regulation and nutritional support can improve ventilator-associated pneumonia in immune function and nutritional status, reduce ventilation time, is helpful to improve the prognosis of the patients.
