连续性肾脏替代疗法在严重挤压伤致急性肾功能衰竭抢救中的应用价值分析

摘    要:目的探讨连续性肾脏替代疗法在严重挤压伤致急性肾功能衰竭抢救中的应用价值。方法选取2010年3月-2012年7月在我院抢救的48例严重挤压伤致急性肾功能衰竭患者,所有患者均给予连续。肾脏替代疗法治疗,统计并分析治疗前后患者的收缩压、舒张压、心率以及呼吸频率等心功能以及BUN、Ser、血钾、血钠等生化指标变化情况。结果急性肾功能衰竭患者治疗后的收缩压、舒张压、心率以及呼吸频率等心功能情况明显优于治疗前,治疗前后差异均有统计学意义(P〈0.05);急性肾功能衰竭患者治疗后的BUN、Scr、血钾、血钠、HC03-及BE等生化指标明显优于治疗前,治疗前后差异有统计学意义(P〈0.05)。结论连续性肾脏替代疗法在严重挤压伤致急性肾功能衰竭患者抢救中能够明显改善患者的心功能和血液生化指标,是治疗急性肾功能衰竭的一种快速有效的治疗方法,值得临床推广应用。

The efficacy of continuous renal replacement therapy in the rescue of a- cute renal failure induced by severe crush injury

Abstract:Objective To investigate the efficacy of continuous renal replacement therapy in the rescue of acute renal failure induced by severe crush injury. Methods From March 2010 to July 2012, 48 patients with acute renal failure induced by severe crush injury in our hospital were chose to study, all patients were treated with continuous renal re- placement therapy, heart function such as systolic blood pressure,diastolic blood pressure, heart rate and respiratory rate,and changes of biochemical markers such as BUN, Scr, serum potassium, serum sodium before and after treatment were analyzed. Results After the treatment, the systolic blood pressure, diastolic blood pressure, heart rate and respira- tory rate, heart function of acute renal failure patients were significantly better than before treatment, the data of pre- treatment and post-treatment were statistically significant differences (P 〈 0.05); after treatment,the BUN, Scr, serum potassium, serum sodium, HCO3- and BE biochemical parameters significantly better than before treatment, the pre- treatment and post-treatment data difference was statistically significant (P 〈 0.05). Conclusion Continuous renal re- placement therapy in severe crush injury induced acute renal failure can significantly improve cardiac function and blood biochemical parameters in patients, and it is a fast and effective treatment for acute renal failure. It is worthy of clinical application.
