摘    要:目的 探讨颅脑创伤后诱发ARDS诊断及治疗方法. 方法回顾性分析2010年10月~2012年10月我科收治的70例颅脑创伤并发ARDS患者临床资料. 结果治疗组抢救成功26例,死亡11例,死亡率为29.73%;对照组救治成功20例,死亡13例,死亡率为39.39%.两组治疗效果及PaO2、PCO2、平均机械通气时间差异显著,具有统计学意义(P<0.05). 结论早期小潮气量高水平正压通气可有效增强颅脑创伤诱发ARDS患者肺顺应性,促进萎陷肺泡扩张,降低死亡率,值得临床推广.

The impact of different levels of low tidal volume ventilation on high pressure on traumatic brain injury induced ARDS

Abstract:Objective To investigate the diagnosis and treatment of traumatic brain injury induced ARDS. Methods The clinical data of 70 patients admitted to our department with traumatic brain injury induced ARDS from October, 2010 to October, 2012 were retrospectively analyzed. Results The 26 patients in the treatment group were successfully rescued. 11 patients died, and mortality was 29.73%. 20 cases were the success of the control group patients, 13 pa- tients died and the mortality rate was 39.39%. Differences of the therapeutic effect and Pa02, PCO2 and average me- chanical ventilation time of the two groups of patients were significantly with statistical significance (P〈0.05). Conclu- sion Early traumatic brain injury-induced respiratory distress syndrome with low tidal volume high levels of positive pressure ventilation can effectively promote the expansion of the patient collapsed alveoli, reduce patients' mortality and is worthy of clinical promotion.
