超敏C反应蛋白与糖尿病肾病的相关性研究

目的探讨超敏C反应蛋白（hs—CRP）与糖尿病肾病（diabetic nephropathy，DN）的相关性，旨在为DN的诊治提供进一步的理论支持。方法将2009年1月～2010年1月在我院人院治疗的80例2型糖尿病患者（T2DM组）及同期来我院进行健康体检者30例作为健康对照组（且排除高血压、糖尿病、肝炎、泌尿系感染等）纳入本研究。根据24h尿微量白蛋白（urinary albumin，MAU）将T2DM患者分为i组：单纯糖尿病组（MAU〈30mg／24h）25例（SDM组）；早期糖尿病肾病MAU组fMAU（30～300）rag／24h130例（EDN组）；临床糖尿病肾病（MAU〉300mg／24h）组25例fCDN组1。检测各组的hs—CRP、HbAlC、Cr表达水平，并进行相关性分析。结果EDN组、CDN组的hs—CRP、HbAlC、Cr均分别明显高于正常对照组，且CDN组hs—CRP、HbAlC、Cr明显高于EDN组，差异有统计学意义（P〈0．05）。DN患者hs—CRP与HbAlC、Cr存在正相关fr=0．286，0．332，P〈0．05）。结论超敏C反应蛋白（hs—CRP）与HbAlC、Cr从不同途径参与了糖尿病肾病的发生发展过程，因此临床通过监测hs—CRP水平对于判断DN的病情及诊治具有重要的临床指导价值。

The correlation of high-sensitivity C-reactive protein and diabetic nephropathy

Objective To investigate the correlation of high-sensitivity C-reactive protein(hs-CRP) and diabetic nephropathy(diabetic nephropathy,DN),and to diagnosis and treatment of DN is designed to provide further theoretical support.Methods January 2009 to January 2010 in our hospital,80 hospitalized patients with type 2 diabetes(T2DM group) and to our hospital for the same period 30 patients healthy as healthy control group(and exclude hypertension,diabetes,hepatitis,urinary tract infection,etc.) included in the study.According to 24h urinary albumin(urinary albumin,MAU) to T2DM patients were divided into three groups: simple diabetic group(MAU <30 mg/24 h) 25 cases(SDM group);early diabetic nephropathy I MAU(30～300) mg/24 h group of 30 patients(EDN group);clinical diabetic nephropathy(MAU>300 mg/24 h) 25 patients(CDN group).Detected in each group of hs-CRP,HbA1C,Cr expression levels,and correlation analysis.Results EDN group,CDN group of hs-CRP,HbA1C,Cr were significantly higher than the control group,respectively,and the CDN group hs-CRP,HbA1C,Cr was significantly higher than EDN group,the difference was statistically significant(P < 0.05).Hs-CRP in patients with DN and HbA1C,Cr a positive correlation(r = 0.286,r = 0.332,P <0.05).Conclusion High-sensitivity C-reactive protein(hs-CRP) and HbA1C,Cr from different sources involved in the development and progression of diabetic nephropathy,the clinical hs-CRP levels by monitoring the DN for the condition judgment and treatment has important clinical value.
