2型糖尿病患者颈动脉粥样硬化危险因素分析

目的探讨2型糖尿病患者颈动脉粥样硬化与其危险因素的关系。方法用菲利浦7500型彩色多普勒超声诊断仪对109例2型糖尿病患者双侧颈动脉进行检查,根据颈动脉内中膜厚度(IMT)及观察颈动脉有无斑块,分成A组(颈动脉IMT1.0mm及无颈动脉斑块)及B组(颈动脉IMT≥1.0mm和/或颈动脉斑块),记录年龄、体重指数、糖尿病病程;测定血糖、血压、血脂、血尿酸、纤维蛋白原。结果两组年龄、收缩压、血糖、总胆固醇、低密度脂蛋白胆固醇、血尿酸、颈动脉内中膜厚度差异有统计学意义(P0.01或0.05)。Logistic回归分析表明,年龄(B=0.052,P0.05)、收缩压(B=0.029,P0.05)、低密度脂蛋白胆固醇(B=1.771,P0.01)、血尿酸(B=0.006,P0.05)是其独立危险因素。结论年龄、收缩压、低密度脂蛋白胆固醇、血尿酸是2型糖尿病患者颈动脉粥样硬化的独立危险因素。

Analysis of Risk Factors for Type 2 Diabetes with Carotid Atherosclerosis

Objective To investigate the relationship between carotid atherosclerosis of type 2 diabetic patients with its risk factors. Methods The carotid intima-media thickness（IMT） was examined in 109 patients with type 2 diabetes by Philips 7500 Color Doppler ultrasonic diagnostic apparatus. The patients were divided into group A（carotid IMT〈 1.0mm and the absence of carotid plaque） and group B （carotid IMT≥1.0mm and/or the plaque）. The differences of the risk factors between two groups were analyzed. Results There were significant differences in age, systolic blood pressure,fasting blood glucose, total cholesterol,low density lipoprotein cholesterol, uric acid and carotid IMT（ P〈 0.01 or P 〈 0.05 ） between two groups. Logistic regression analysis showed that age （ B = 0.052, P〈 0.05 ）, systolic blood pressure （B = 0.029, P〈 0.05 ）, low density lipoprotein cholesterol （ B = 1.771, P〈0.01 ）and uric acid （ B =0.006, P〈0.05 ）were the risk factors for type 2 diabetes with carotid atherosclerosis. Conclusion Age, systolic blood pressure,low density lipoprotein cholesterol, uric acid are risk factors for type 2 diabetes with carotid atherosclerosis.
