全麻和单侧重比重腰硬联合麻醉全髋关节置换术的临床比较

摘    要:目的探讨全身麻醉与单侧重比重腰硬联合麻醉对全髋关节置换术患者的麻醉效果及安全性,为该类病人的临床麻醉提供参考。方法将106例老年全髋关节置换术患者随机分为全麻组(A组)和硬腰联合麻醉组(B组),观察麻醉前、麻醉后SBP、DBP、HR,并进行对比分析。结果B组麻醉效果(90.6%)优于A组(84.9%),但差异无统计学意义(P〉0.05)。B组深静脉栓塞、肺部感染、术后认知功能障碍的发生率低于A组,差异有统计学意义(P〈0.05)。结论腰硬联合麻醉和全身麻醉均能满足手术需要,但采用腰硬联合麻醉方式并发症少,更优于后者。

Unilateral weighty specific gravity combined spinal epidural anesthesia and general anesthesia on total hip replacement :A clinical analysis of 53 cases

Abstract:Objective To compare and analyze the effects of general anesthesia and unilateral weighty specific gravity com- bined spinal-epidural anesthesia (CSEA) in total hip replacement. Methods A total of 106 patients were divided into general anesthesia group (group A, n = 53) and combined spinal epidural anesthesia group (group B, tt = 53). The anesthetic effect,complications,HR, SBP,and DBP of patients during and after operation in the two groups were recorded. Results Al- though the anesthetic effect in group B (90.6%) was better than that in group A (84.9%), but there was signifient differ- ence (P 〉 0.05). About complications such as deep vein thrombosis,lung infection, postoperative cognitive dysfunction after operation,group B significantly less than in group A(P 〈 0.05). Conclusion Combined spinal-epidural anesthesia and gen- eral anesthesia can meet the surgical needs,but CSEA has fewer complicationsland better than general anesthesia.
