彩色多普勒超声测定肝硬化门脉血流的价值

目的探讨彩色多普勒超声检查对肝硬化患者门脉血流改变的临床意义。方法收集2003年8月～2008年3月我院临床诊断肝硬化门脉高症的80例患者，肝功能Child—Pugh分级：A级27例，B级30例，C级23例，测量门静脉（PV）内径、脾静脉（SV）内径、平均血流速度（cm／s）和血流量（ml／min）。结果不同肝功能级别组与对照组比较血管内径明显增宽，血流速度明显减慢，血流量明显增加（P〈0．01）。不同肝功能级别组间随着肝功能损害程度的加重、血管内径增大及血流速度减慢也有显著改变（P〈0．05），但血流量参数无显著差异（P〉0．05）。结论多普勒超声多能动态反映肝硬化患者门脉血流改变是评价肝功能、肝硬化病变程度较敏感指标。

The Value of Portal Blood Flow by Color Doppler Ultrasonography

Objective To study of portal blood flow in patients with cirrhosis by color Doppler ultrasound. Methods 80 cases with Child-Pugh classification ： Grade A（n=27）,Grade B（n=27）,Grade C（n=27）,measurement of portal vein （PV） diameter,splenic vein （SV） diameter,the average flow velocity （cm /s）and blood flow （ml/min. Results The liver function-level group and the control group was obviously enlarged blood vessel diameter,blood flow velocity significantly,blood flow was significantly increased （P〈0.01）. Different levels of liver function group with the degree of liver damage increased vascular diameter and flow velocity increases have slowed down significantly （P〈0.05）,but the blood flow parameters of no significant difference （P〉0.05）. Conclusion Doppler ultrasound can be more dynamic portal blood flow in patients with cirrhosis to reflect changes in the evaluation of liver function,cirrhosis of the liver lesions more sensitive indicators
