摘    要:目的探讨外源性褪黑素对重症急性胰腺炎(SAP)相关肾脏损伤的保护作用及机制。方法雄性SD大鼠90只随机分为正常对照组(N组)、SAP并肾损害组(S组)、褪黑素干预组(M组)。光镜下观察胰腺、肾组织病理改变,对肾组织病理损害评分;检测各组大鼠血清淀粉酶(AMY)、肌酐(Cr)、丙二醛(MDA)含量及肾细胞凋亡指数。评估褪黑素对SAP并肾脏损伤的保护作用与脂质过氧化、肾细胞凋亡的相关性。结果S组随造模时间延长病理损伤逐步加重,M组相同时间点。肾脏病理损害均较S组减轻(P〈0.05);与S组比较,M组相同时间点血清AMY、Cr、MDA含量及肾细胞凋亡指数均降低(P〈0.05);S组及M组中MDA与血清Cr、肾细胞凋亡指数均呈正相关(P〈0.05)。结论褪黑素对SAP相关肾脏损伤有保护作用,其机制可能是通过抗脂质过氧化,抑制肾脏细胞凋亡来实现。

Protective Effect and Mechanism of Melatonin on Acute Severe Pancreatitis Associated with Renal Injury in Rats

Abstract:Objective To investigate the protective effect and related mechanisms of exogenous melatonin on severe acute pancreatitis (SAP)associated with renal injury in rats. Methods Ninety SD rats were equally divided into three groups randomly:normal control group(group N),SAP associated with renal injury group(group S) and melatonin intervention group(group M),whose pathologic changes of kidney were observed and graded under microscope;Then the level of serum amylase(AMY),creatinine(Cr),malondialdehyde(MDA) and the apoptosis index of renal cell in rats were measured. Finally,the correlation among protective effect of melatonin on renal injury in SAP,lipid peroxidation and renal cell apoptosis were assessed. Results The pathological lesion in group S was gradually aggravated with extension of molding time ;The renal pathological lesion in group M at the same time point was more remissive than that in group S(P〈0.05). Compared with group S at the same time point,the expression level of serum AMY,Cr,MDA and apoptosis index in group M were decreased(P〈0.05). In group S and M,the expression level of MDA were positively correlated with Cr and apoptosis index respectively (P〈0.05). Conclusion The effects of melatonin has a protective function on renal injury in SAP,its mechanism exerts of this effect may depend on anti-lipid peroxidation and decreasing renal cell apoptosis in kidney.
