基于功能性磁共振成像技术的针灸治疗抑郁症中枢机制研究进展

抑郁症以持久而显著的心境低落为主要临床特征,是心境障碍的主要类型,也是脑功能障碍性疾病之一。针灸治疗抑郁症的疗效己受到国内外的充分肯定,但其中枢机制尚未形成统一的标准,普遍认为是通过调节边缘系统皮质纹状体丘脑神经通路来实现。近年来有学者应用功能性磁共振成像技术对针灸治疗抑郁症的中枢机制进行了研究,笔者拟从抑郁症的相关脑功能网络异常和针灸治疗抑郁症脑功能网络两方面的f MRI研究进展进行综述。结果显示目前针灸治疗抑郁症脑功能网络仅局限于单个或多个独立的脑功能网络,尚未形成科学、规范、完整的脑功能网络机制。笔者建议应着重研究脑功能网络之间的整合作用机制,同时要深入探讨针灸治疗抑郁症脑功能网络的f MRI机制,以期能进一步证实其稳定性及可重复性,为针灸治疗抑郁症的研究做指导。

Research progress on the central mechanism of acupuncture and moxibustion in the treatment of depression based on functional magnetic resonance imaging

The main clinical features of depression are persistent and significant depression, which is one of the main types of mood disorders, and is also one of the diseases of brain dysfunction. The therapeutic effect of acupuncture and moxibustion on depression has been fully affirmed at home and abroad. However, there is no uniform standard in terms of mechanism at present. It is generally believed to be achieved by regulating the cerebral pathway of the cortical colliculus of the limbic system. In recent years, some scholars have studied the central mechanism of acupuncture and moxibustion for the treatment of depression with functional magnetic resonance imaging (fMRI). I will review the progress of fMRI research on depression related brain functional network abnormalities and acupuncture treatment of depression brain functional network. The results showed that the brain functional network of acupuncture and moxibustion treatment for depression was limited to single or multiple independent brain functional networks. There is no scientific, normative, and complete brain functional network mechanism. The author suggests that it should be focused on the mechanism of integration between brain function networks. At the same time, the fMRI mechanism of acupuncture and moxibustion for the treatment of depressive brain function network should be explored in order to further confirm its stability and repeatability, and provide guidance for the study of acupuncture and moxibustion in the treatment of depression.
