痰罗氏培养结核菌阳性、血 T-spotTB 阴性常见原因分析

目的：探讨肺结核罗氏培养结核菌阳性患者而血 T-spotTB 阴性的原因。方法对2013年1月1日—2014年1月1日首都医科大学附属北京胸科医院收治的1800例且病历资料完整的肺结核患者进行回顾分析，观察痰菌阳性（罗氏培养阳性），T-spotTB 阴性患者的发生原因。结果1800例中，发生肺结核罗氏培养结核菌阳性患者而血 T-spotTB 阴性患者40例。结论肺结核痰菌阳性患者出现血 T-spotTB 阴性的常见原因与合并低蛋白血症、贫血、结缔组织疾病、癌症、毒性乙型肝炎等因素相关。

Common cause analysis of negative blood T-spotTB in patients with positive tubercle bacillus by Roche culture with phlegm

Objective To explore the cause of negative blood T-spotTB in patients with positive tubercle bacillus by Roche culture with phlegm. Methods The 1800 patients with pulmonary tuberculosis who were admitted in Bei-jing Chest Hospital Affiliated to Capital Medical University from January 01,2013 to January 01,2014 with complete medical records were analyzed to find out the reason for T-spotTB being negative while tubercle bacillus by Roche cul-ture being positive. Results Within the 1800 patients,there were 40 patients who had negative blood T-spotTB and positive tubercle bacillus by Roche culture with phlegm. Conclusion The common cause of negative blood T-spotTB in patients with positive tubercle bacillus by Roche culture with phlegm is associated with hypoproteinemia,anemia, connective tissue disease,cancer,and toxic hepatitis B,and etc.
