158例宫颈涂片液基细胞技术及TBS系统检测的临床分析

摘    要:目的评价液基细胞技术(LCT)及Bethesda系统(TBS)对宫颈病变临床诊断的价值。方法2007年5月~2008年5月对我院第2门诊妇产科就诊的158例患者行LCT检查和TBS细胞学分类诊断,对LCT检查发现异常的患者行阴道镜病理检查和HPV(人乳头瘤病毒)-DNA检测。结果158例涂片中检出异常涂片26例(16.4%),其中不典型鳞状细胞(ASCUS)23例(16%),低度鳞状上皮内病变(LSIL)2例(1.3%),高度鳞状上皮内病变(HSIL)1例(0.7%),鳞状细胞癌(SCC)0例,子宫内膜腺癌0例。LCT与阴道镜病理检查和HPV检测的符合率分别为50%、83%。结论LCT技术应用于宫颈涂片配合阴道镜检查、活检和HPV-DNA检测,是筛查和诊断子宫颈癌前病变和癌变的可靠方法。

Abstract:Objective Evaluating the value of the liquid-based cervical smear technology(LCT) and the Bethesda system( TBS) used in the clinical diagnoisis on the pathological changes of cervical lesion. Methods In the second department of gynecology outpatient in our hospital from May. 2007 to May. 2008, there are 158 cases which respectively take LCT inspection and the Bethesda system(TBS), the patients taking LCT inspection find out abnormality, who will take inspection of colposcopy and biopsy HPV (human nipple HPV )- DNA. Results The test shows that there are 26 cases of abnormalities in the 158 cases ( 16.4 % (, a typical squamous cell of undetermined significance (ASCUS) take up 23 cases ( 16 % ), low-grade squamous intraepithelial lesions( LSIL ) take up 2 cases ( 1.3 % ), high- grade squamous intraepithelial lesions( HSIL)take up 1 cases (16% ) ,without any patient have Squamous cell lung and invasive cervical cancer. The testing matching rate of LCT and biopsy is 50 %, The testing matching rate of LCT and HPV is 83 %. Conclusion It is a reliable way to prevent and diagnose pathological before change to cervical cancer and have changed to cancer by using LCT technology in cervical smear conduct detection, diagnosis and screening with colposeopy,biopsy and HPV-DNA.
