数字X线牙片诊断下(牙合)第一磨牙牙根纵裂的离体研究

摘    要:目的利用离体牙评价X线片在下第一磨牙牙根纵裂诊断中的作用。方法采用平行投照对18颗下第一磨体牙拍摄不同角度数字X线片,4名观察者独立评价X线片,计算各观察者之间的一致性,选取一致性最好的2名观察者的结果纳入最后评价。结果同时观察5张X线片比单独观察0°X线片的一致性好,X线片诊断下第一磨牙牙根纵裂敏感度和特异度好(Sen=77.8%,Spe=88.9%)。结论数字X线片能为下第一磨牙牙根纵裂的临床诊断提供重要的依据。

Abstract:Objective To evaluate the contribution of digital radiogram in diagnosis of vertical root factrue in mandibular first molar.Methods Eighteen extracted mandibular molar teeth were radiographed in mesio-distal planes with paralleling technique Four examiners assessed all the films alone to judge if there was a root vertical root fracture.Then calculated the Kappa value between each examiner,chose the two examiners' results who had the highest Kappa value as te final results.Results The results examining five graphs at the same time got higer Kappa scores than examining only 0 angulation.The sensitivity and specialty of ardiograph are satisfactory (Sen=77.8%,Spe=88.9%).Conclusion Radiograph can provide essential evidence for the clinical diagnosis of the vertical root fracture in mandibular first molar teeth.
