老年高血压并微量清蛋白尿与颈动脉内膜-中层厚度的相关性分析

摘    要:目的探讨高血压病并微量清蛋白尿与颈动脉内膜中层厚度及血尿酸、血脂的关系。方法 142例老年高血压患者按尿清蛋白水平分为两组,正常组75例,微量清蛋白尿组67例。对两组的颈动脉内膜-中层厚度(IMT)、斑块发生率、尿微量清蛋白(MAU)水平、血中尿酸、(UA)总胆固醇(TC)、低密度脂蛋白(LDL-C)浓度进行分析和比较。结果微量清蛋白尿组的IMT、斑块发生率、MAU水平、血UA、TC、LDL-C浓度均高于正常清蛋白尿组(P<0.05);微量清蛋白尿组IMT和血尿酸浓度与MAU水平呈正相关(P<0.05)。结论老年高血压并微量清蛋白尿患者多合并高尿酸及高脂血症,微量清蛋白尿与动脉粥样硬化发生及程度密切相关,可作为心脑血管病变早期预测的重要指标。

Abstract:Objective To evaluate the relationship between microalbuminuria(MAU) and carotid intima-media thickness(IMT),and uric acid(UA),serum total cholesterol(TC),LDL-C in elder patients with essential hypertension(EH).Methods 142 patients with EH were divided into two groups according to the MAU level.75 patients were normal albuminuria group,67 patients were MAU group.The IMT and the level of MAU,UA,TC,LDL-C in serum were observed and compared between the two groups.Results The IMT,the incidence of plaque,the level of MAU,concentration of UA、TC、LDL-C in MAU group were higher than those in normal albuminuria group(P<0.05).There were positive correlations between The level of MAU and C-IMT,the concentration of UA in MAU group(P<0.05).Conclusion The incidence and extent of atherosclerosis is closely related to MAU in elder patients with EH,therefore,the level of MAU may be used as an early significant index for the evaluation of development of renal disease and cardio-cerebro vascular disease in the EH patients.
