~(99m)Tc-DTPA测定肾小球滤过率在慢性肾功能不全中的应用

摘    要:目的 研究99mTc DTPA清除率在肾功能不全 (CRI)分期的和确定透析指针的意义。方法  48例不同分期的CRI患者用99mTc DTPA肾清除率方法测定肾小球滤过率 (GFR ) ,其结果与生化法测定的内生肌酐清除率进行比较。结果 GFR与Ccr与呈正相关 ,GFR与Scr呈负相关 ,GFR与BUN间无相关性。结论 由于生化方法测定Ccr与Scr受很多因素的影响 ,因此不如99mTc DTPA测定清除率准确 ,99mTc DTPA测定清除率不仅准确 ,还有敏感、快速、重复性能好的优点。

Application of ~(99m)Tc-DTPA clearance in chronic renal insufficienct.
Abstract:Objective To go further into the significance of different stages of chronic renal insufficiency(CRI)and determine the indication of dialysis.Methods The glomerular filtration rate(GFR) for 48 patients with different stages of CRI was measured by 99m Tc DTPA renal clearance,and the results were compared with the outcomes of serum creatinine clearance(Ccr)measured by biochemical method.Results There existed both a positive correlation between GFR and Ccr and a negative correlation between GFR and serum creatinine (Scr).No correlation was obserred between GFR and blood urea nitrogen(BUN).Conclusion Because the measurement of Ccr and Scr by biochemical method is influenced by many factors,the Ccr measured biochemically is less accurate than the GFR measured by 99m Tc DTPA.The measurement of GFR by 99m Tc DTPA renal clearance not only shows satisfactory accuracy but also has the advantages of sensitivity,rapidness and reproducibility.
