成都龙泉驿区小儿肺炎细菌分布及耐药状况分析

摘    要:目的了解我院小儿肺炎细菌分布及耐药状况,指导早期经验性抗生素应用。方法统计分析了2007年1月~2009年12月我院儿科242例肺炎住院患儿取深部痰培养结果。结果共分离培养出病原菌152株,阳性率62.8%,其中革兰阴性菌100株(65.8%);革兰阳性菌47株(30.9%);真菌5株(3.3%)。革兰阴性菌以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、鲍曼不动杆菌为主,较敏感的抗生素为亚胺培南、哌拉西林/他唑巴坦、头孢哌酮/舒巴坦、阿米卡星、环丙沙星、第四代头孢菌素,对氨苄青霉素的耐药性最高。革兰阳性菌以金黄色葡萄球菌和表皮葡萄球菌为主,对青霉素的耐药率达100%,对万古霉素、苯唑西林敏感,对哌拉西林/他唑巴坦和头孢哌酮/舒巴坦也有较高敏感性。结论我院小儿肺炎的病原菌以革兰阴性菌为主,其耐药性较高。

Abstract:Objective To investigate the bacterial distribution and drug resistance from children with pneumonia,and to analyce the revsons of antibivties resistance of the parthogens.Methods The sputum calture reports of 242 children hospitalized with pneumonia from January 2007 to December 2009 were wllected and analyzed.Results A tolal of 152 pathogenic strains (62.8%) were identified,including 100(65.8%);Cram-negative bacilli 47(30.9%);Cram-postitvie bacilli,and 5(3.3%) fangi.Escherichia coli,klebsiella pneumoniae,Pseudomonas aeruginosa,and Acinetobacter baumannic were common cutured Cram-negative bacilli.Most strains of Cram-negative bacilli were susceptible to mropenen,piperacillin/tazobactam,ceoperazone/sulbactam,amikacin,cprofloxacin,the foarth generation cealosporin.Staphylocouus aureus and straphylococcus epidermidis were common in the cultured Cram-positive bacilli.Staphylococcus aureus and Straphylococcus epidermidis were susceptible to vancomycin prostaphlin piperacillin/tazobactam,cebfoperazone/sulbactam but were resistant to penicillin.Conclusion Cram-negative bacilli predominate the pathogens of pneumo in neonates.Bacterias presented high antibiotic resostance.
