老年心力衰竭患者血浆TNF-α、TGF-β_1变化的意义

目的 :探讨老年充血性心力衰竭 (CHF)患者血中肿瘤坏死因子 -α(TNF -α)和转化生长因子 β1 (TGF -β1 )的变化及其临床意义。方法 :用双抗体夹心ELISA法测定 60例CHF患者及 2 0例健康人血浆中TNF -α和TGF -β1的浓度。超声心动图测量左室射血分数 (LVEF)及左室舒张末期内径 (LVEDd)。结果 :CHF患者血浆中TNF -α水平明显高于对照组 (P <0 .0 5或P <0 .0 1) ,且随着心衰程度的加重 ,TNF -α水平呈进行性增高 ;TGF -β1 水平在心功能Ⅲ、Ⅳ级组中明显高于对照组 (P <0 .0 5或P <0 .0 1) ,而心功能Ⅱ级组与对照组相比无显著性差异。TNF -α、TGF-β1 与LVEF呈负相关 (r =-0 .5 13 ,r =-0 .45 3 ,P <0 .0 1)。结论 :TNF -α和TGF -β1 的升高可作为CHF严重程度的判断指标。CHF患者血中高浓度的TNF -α和TGF -β1 参与了CHF的发生发展

The Changes and Importance of TNF-α,TGF-β_1 in Old Patients with Congestive Heart Failure

Objective:To explore the changes and importance of tumor necrosis factor-α(TNF-α),transforming growth factor-β 1(TGF-β 1)in patients with congestive heart failure (CHF). Methods:The plasma levels of TNF-α,TGF-β 1 were examined by ELISA in 60 cases with CHF and 20 normal conrtols. The left ventricular ejective fraction (LVEF) were examined by echocardiography.Results:Plasma levels of TNF-α were increased in CHF compared with controls(P<0 05 or 0.01). The more severer CHF, the more higher plasma TNF-α levels. TGF-β 1 levels were elevated in NYHA function class Ⅲ and Ⅳ compared with controls (P<0.05 or 0.01),but there were no difference between class Ⅱ and controls. TNF-α,TGF-β 1 were negatively correlated with LVEF(r=-0.513,r=-0.453,P<0.01).Conclusions:Plasma levels of TNF-α and TGF-β 1 can be an important sign in clinical judgement of the severity of CHF. TNF-α and TGF-β 1 may play an important role in the progression of CHF.
