| 摘 要： | 目的探讨以中枢神经系统症状为首发表现的获得性免疫缺陷综合征(AIDS)的临床特征及预后,提高临床医师对这部分患者的诊断识别能力,防止临床漏诊和误诊。方法回顾性分析2009～2010年云南省传染病专科医院感染科收治的500例艾滋病患者的临床资料,其中80例患者以中枢神经系统表现为首发症状。结果 80例患者中AIDS痴呆综合征5例,结核性脑膜炎15例(包括结核性脑膜脑炎),隐球菌脑膜炎30例,弓形虫脑病20例,脑梗死4例,巨细胞病毒神经根炎3例,进行性多灶性白质脑病3例。所有病例出现中枢神经系统表现后行人类免疫缺陷病毒抗体检测阳性,诊断为艾滋病。结论以中枢神经系统症状为首发症状的AIDS患者病情复杂,预后差,病死率高,早期诊断及治疗可以使患者病死率下降。
