颈椎后路椎管扩大成形术后轴性症状的研究进展

单开门椎管扩大成形术治疗多节段脊髓型颈椎病近期和远期均取得了良好的效果。但是,许多患者在此手术后出现颈椎活动受限、鹅颈畸形、遗留或出现长期的颈项部及肩背部疼痛、僵硬和肌肉痉挛等轴性症状,发生率达45%～80%,持续时间可长达十余年。术后轴性症状发生的确切机制尚不完全清楚。目前认为主要和颈椎后伸机制的破坏及颈椎活动度的减少有关。目前尚无治疗轴性症状的特效方法,众多学者通过对传统颈后路C3～C7单开门椎管扩大成形术进行改良来减少轴性症状的发生。

Progress in Axial Symptom after Cervicallaminoplasty

Laminoplasty have achieved good efficacy either in short-term or long-term in the treatment of multi-segmental cervical spondylotic myelopathy.However,many patients after cervical spine surgery suffer kinds of complications such as limited mobility,swan-neck deformity,shoulder and neck pain,stiffness and muscle spasms or other axial symptoms.The incidence of patients with evident axial symptoms was 45%～80%.Although the exact pathology of axial symptoms is unclear,many surgeons suspect that they are related to the damage of cervical extension mechanism and the decrease of cervical activity.There is no efficient treatment for axial symptoms nowadays,many researchers reduce the incidence of axial symptoms by improving the traditional posterior C3～ C7 laminoplasty.
