介入治疗难治性产后出血5例分析

目的观察子宫动脉栓塞术治疗难治性产后出血的效果。方法采用seldinger技术对5例难治性产后出血患者行经皮股动脉置管造影确定子宫动脉走向,在彩超或X线电视影像技术监视下,灌注明胶海绵颗粒栓塞血管,治疗出血。结果 5例均一次手术栓塞成功。手术时间短、止血快、保留子宫及功能。无一例术后发生下肢动脉血栓形成,手术恢复良好。结论动脉栓塞术治疗产后难治性出血效果可靠,并保留子宫及功能。

Analyses of Interventional Treatment of Five Cases with Intractable Postpartum Uterine Hemorrhage

Objective To observe the effects and the methods of arterial medical embolization to cure the intractable postpartum uterine hemorrhage.Methods With the help of the Seldinger technique,the angiography of femoral artery was performed to position the uterine artery in five patients.Under the monitoring of color Doppler ultrasound and television image,gelatin sponge particles were injected into the uterine artery to control its bleeding.Results All the five patients were successfully cured postoperatively.Such method took shorter time and controlled the bleeding more rapidly,able to preserve the uterus and its function.No patient had lower limb arterial embolism postoperatively whereas all of them had recovered well.Conclusion The arterial embolism was reliable for intractable postpartum uterine hemorrhage,able to preserve the uterus and its function.
