腹腔镜与开腹子宫肌瘤切除术的对比研究

目的:评价腹腔镜子宫肌瘤切除术的临床应用价值。方法:选取行腹腔镜子宫肌瘤切除术(LM)40例,与同期经腹子宫肌瘤切除术(TAM)43例进行对照分析,对手术时间、术中出血量、术后肛门排气时间及术后住院日等进行对比。结果:LM组手术时间(96.24±36.35min),与TAM组(60.12±15.32min)比较,LM手术时间较TAM长,差异有显著性(P<0.01);术后肛门排气时间(18.62±5.61h)及术后住院日(5.54±1.80d)均较TAM组肛门排气时间(22.45±8.32h)及术后住院日(7.62±2.07d)缩短,差异有极显著性(P<0.01);LM组术中出血量(124.88±54.46ml)与TAM组(112.67±66.48ml)无显著统计学差异(P>0.05)。结论:腹腔镜子宫肌瘤切除术具有创伤小、术后恢复快等优点,是一种安全、可行的理想术式。

A comparative research between laparoscopic and trans-abdominal myomectomy

0bjective：To investigate the clinical value of laparoscopic myomectomy. Methods：A retrospective comparative research was made on clinical data（such as operation time, blood loss , time of passage of gas by anus ,hospital stay） of 40 cases of laparoscopic myomectomy（LM） and 43 cases of trans-abdominal myomectomy （TAM） from January 2006 to January 2009. Results：Only one case was converted to laparotom. The LM Group had significantly longer operation time（96.24±36.35） min than the TAM Group（60.12±15.32 ） min （P0.O1）,shorter hospital stay（5.54±1.80） d than the TAM Group（7.62±2.07） d （P0.O1）,shorter time of passage of gas by anus（18.62±5.61） d than that of the TAM group （7.62±2.07） d . There were statistical differences between two groups （P0.01）. The blood loss was similar in the LM Group（124.88±54.46）ml and that in the TAM Group（112.67±66.48）ml （P0.O5）.Conclusion： Laparoscopic myomectomy has advantages of fast recovery,short hospital stay,and low complication rate,LM can be an ideal procedure for myomectomy.
