经尿道手术和开放手术治疗前列腺增生症746例对比研究

目的研究经尿道前列腺切除术(TURP)、经尿道前列腺电气化术(TUVP)和耻骨上经膀胱前列腺摘除术(SPP)治疗前列腺增生症的临床疗效。方法对746例良性前列腺增生症(BPH)手术患者行TURP300例,TUVP83例,SPP363例,回顾分析并比较各组手术时间、出血量、输血量、疗效及并发症等。结果TURP、TUVP和SPP组手术前后IPSS、QOL、RUV、Qmax有显著性差异(P<0.05),三者疗效比较无显著差异(P>0.05)。SPP组出血量及输血量,膀胱冲洗时间、置管时间及平均住院时间均多于或长于TURP、TUVP组(P<0.05),TURP、TUVP两组上述指标比较无显著性差异(P>0.05)。结论TURP、TUVP和SPP均是治疗BPH的有效方法,手术创伤SPP最大,TURP最小。联合应用TURP和TUVP,可提高手术疗效。

A comparative study and evaluations on the results of transurethral electroresection,transurethral vaporization and open prostatectomy for the treatment of BPH(a report of 746 cases)

Objective: To study and compare the effective of the transurethral electroresection(TURP),transurethral vaporization of the prostate(TUVP)and suprapubical transvesical prostatectomy(SPP) for the treatment of BPH .Methods: 746 cases of BPH were treated with TURP (n=300),TUVP(n=83) and SPP(n=363), and the therapeutic effects were evaluated. Results: There are significant improvements in symptoms and physical signs in each group after the procedure(P>0.05).The aueraye amount of bleeding in TURP and TUVP group were less than that in SPP group. The mean indwelling catheter time and resident time in TURP and TUVP group were shorter than that in SPP group.Conclution:TURP,TUVP and SPP were all effective for the treatment of prostate. SPP has the most operation injury, while TURP has the least. Combination of TURP and TUVP can improve the effect.
