经皮超声引导下肺外周型占位性病变穿刺活检60例

目的:探讨经皮超声引导下肺外周型占位性病变穿刺活检的临床应用价值。方法:对60例经X线及CT等影像学检查并为超声能显示的肺占位性病变的患者,进行超声引导下穿刺及病理组织检查确诊。结果:良性18例,恶性42例,经与手术切除后病理结果对照,组织病理学符合者53例(89.8%),有4例假阴性(6.84%),3例为坏死组织,未获得明确病理诊断结果。60例中,用18G活检针37例,能获得明确组织病理学结果30例,6例因标本破碎量少而影响诊断;用21G活检针23例,19例获得了明确病理诊断结果。发生并发症5例(8.3%),其中气胸2例,轻度咯血或痰中带血3例。结论:经皮穿刺肺活检诊断阳性率达80%～90%。适用于经体检、胸部X线片、痰检及纤维支气管镜等检查仍未能明确诊断的肺部外周型病灶。

Objective：To investigate the clinical volue of percutaneous ultrasound-guided lung biopsy. Methods： 60 patients with X-ray, CT and ultrasound examination to show the lung lesions of patients, ultrasound-guided puncture inspection, a pathological diagnosis. Results： 18 cases of benign and malignant 42 cases, with the pathology results after surgical resection histopathology, with 53 cases （89.8%）, four official holidays negative （6.84 percent）, two cases of necrotic tissue, could not be clearly pathological Diagnosis. 60 cases, with 18 G needle biopsy in 37 cases, can be clearly pathological results of 30 cases, six cases were broken due to less affected diagnosis by 21 G biopsy needle in 23 cases, 19 cases were a clear diagnosis results. Complications occurred in 5 cases （8.3M）, including two cases of pneumothorax, mild bemoptysis three cases of blood or sputum. Conclusion： Diagnosis rate of percutaneous lung biopsy is 80% to 90%. Apply mainly to： physical examination, chest film, smear-and fiber, such as bronchoscopy examination failed to clear diagnosis of pulmonary lesions.
