高危型HPV检测联合液基细胞学在宫颈癌前病变筛查中的应用

目的:探讨高危型人乳头瘤病毒(HPV)检测联合液基细胞学检测(LCT)在宫颈癌前病变筛查中的应用。方法:对18848例门诊受检者的宫颈脱落细胞标本采用液基细胞技术进行检测,细胞学异常(ASCUS及以上病变)的1691例患者的宫颈脱落细胞标本采用美国Digene公司杂交捕获仪(HC2)检测13种高危型HPV病毒的感染情况。结果:18848例LCT受检者,共发现异常1691例(不包括普通炎症改变)占8.97%,其中ASCUS1044例、LISL416例、HISL192例、SCC39例,分别占5.54%、2.21%、1.02%、0.21%。1691例细胞学异常的受检者中高危型HPV阳性1072例,阳性率63.39%;其中AUSCS、LISL、HISL高危型HPV阳性率分别为53.83%、70.43%、93.23%,显示癌前病变与高危型HPV感染密切相关。结论:高危型HPV检测联合液基细胞学检测能及时有效地发现异常细胞和宫颈癌的诱因,是一种合理而有效的宫颈癌筛查方案。

The application of combining HPV-DNA checking and liquid -based cytology test in cervical screening of precancerous lesions

Objective ：To discuss the application of combining human papilloma virus （HPV） test and liquid-based cytological test in detection of cervical precancerous lesions. Methods：18 848 cases were detected by liquid-base cytology test （LCT）,1691 cases with abnormal cytology results （≥ASCUS） were detected the infection of high risk type of HPV by the hybrid capture I assay （HC2）. Results： There were 1691 cases with abnormal cytology results in 18 848 samples with the rate of 8. 97%. In 1 691 cases with abnormal cytology results, 1 072 cases were made high risk type of HPV infection with the positive rate of 63. 39% ;the rate of infection in AUSCS,LISL and HISL was 53.83%, 70. 43%, 93.23% respectively. The fact indicates cervical dysplasia and high risk type of HPV infection were close correlation. Conclusion： It was the optimal approach for diagnosis of early cervical cancer and precancerous lesions of combination of high risk type of HPV and liquid-base cytology test.
