十二指肠肿瘤50例外科诊断与治疗

目的探讨十二指肠肿瘤的临床诊断及外科治疗效果。方法回顾性分析2008年3月—2009年5月期间于该院就诊的十二指肠肿瘤患者50例相关资料。结果胰十二指肠切除40例,术后1、2、5年生存率分别为77.5%(31/40)、60.0%(24/40)、50.0%(20/40)。十二指肠乳头癌局部切除,胰胆管引流术5例。术后16个月死亡1例,其余均生存至今(超过3年)。十二指肠节段性切除患者2例分别于术后12个月、14个月复发死亡。结论十二指肠肿瘤患者临床症状会因部位的变化而不同,但缺乏特异性。治疗方法应当依据诊断结果进行选择,以胰头十二指肠切除为主,但术后死亡率仍较高,应当结合患者的基本情况。

Diagnosis and Surgical Treatment of 50 Cases of Duodenal Tumor

Objective To study the clinical diagnosis and surgical treatment of duodenal tumor. Methods The data of 50 cases of patients with duodenal tumor admitted in our hospital from March, 2008 to May, 2009 were analyzed retrospectively. Results Out of the 50 cases, 40 cases received duodenopancreatectomy, the survival rates of 1 year, 2 years and 5 years postoperative were 77.5% （31/40）, 60.0%（24/40）and 50.0%（20/40）. 5 cases received local excision of duodenal papillary carcinoma and pancreatic and bile duct drainage. 1 case died 16 months after the operation, the rest are surviving （over 3 years）. 2 cases received duodenal segmental resection, but died 12 months, 14 months after the operation for the recurrence of the disease. Conclusion The clinical symptoms of patients with duodenal tumor vary by location, bat lack specificity. The treatment should be based on the diagnosis and the basic conditions of the patients. In general, pancreatoduodenectomy is the main way of treatmen4 but postoperative mortality is still high.
