低龄与高龄孕妇在孕中期唐氏综合征产前筛查中的筛查效率比较

目的：比较低龄及高龄孕妇在孕中期唐氏综合征产前筛查中的筛查效率。方法整群收集该院2008年1月1日—2014年11月1日间55885例唐氏综合征产前筛查及随访数据，根据孕妇预产期年龄将其分为预产期年龄≥35周岁组以及预产期年龄<35周岁组，比较两组唐氏综合征的发病率，以及对唐氏综合征的检出率、特异度、阳性预测值及阴性预测值。结果高龄孕妇组唐氏综合征的发病率明显高于低龄孕妇组(28.70/万vs 3.14/万)，而在唐氏综合征检出率(100% vs 60.00%)、特异度(78.61% vs 93.60%)、阳性预测值(3.70% vs 0.56%)及阴性预测值(100% vs 99.99%)，两组差异无统计学意义(P>0.05)。结论高龄孕妇组唐氏综合征的发病率明显高于低龄孕妇组，而在唐氏综合征检出率、特异度、阳性预测值及阴性预测值，两组差异无统计学意义。

Comparison of the Efficiency of the Second Trimester Down’s Syndrome Screening Test in Young and Advanced Maternal Age Pregnant Women

Objective To compare the screening efficiency of the second trimester Down’s syndrome screening test in young pregnant women and that with advanced maternal age. Methods Group selection creening and follow-up data of 55 885 cas-es from January 1 2008 to November 1 2014 were collected. The incidence rates, detection rates, specificity and positive predictive values and negative predictive values for Down’s syndrome, in the two group (i.e. the due date maternal age<35 group, and the due date maternal age ≥35 group) were calculated and compared. Results The incidence rates of Down’s syndrome were significantly higher in due date maternal age ≥35 group than that in due date maternal age <35 group (2.87‰ vs 0.314‰), while the detection rates(100% vs 60.00%), specificity(78.61%vs 93.60%), positive predictive val-ues(3.70%vs 0.56%)and negative predictive values (100% vs 99.99%)of the test for Down’s syndrome in two groups were not found significantly different. Conclusion The incidence rates of Down’s syndrome were significantly higher in due date maternal age≥35 group than that in due date maternal age<35 group, while the detection rates, specificity, positive predic-tive values and negative predictive values of the test for Down’s syndrome in two groups were not found significantly different.
