早期肺癌患者呼气中痕量挥发性有机化合物的定量检测

摘    要:目的应用固相微萃取-气相色谱/质谱技术对正常人及早期肺癌患者呼出气体中挥发性有机化合物成分进行定量检测并建立其技术平台。方法用Tedlar采样袋收集正常人和Ⅰ期肺癌患者呼气样品各10份,经固相微萃取浓缩后上样气相色谱/质谱进行定量检测,以标准品为对照,检测结果用Wilicoxon秩和检验统计。结果在呼气样品中可检测出74~114种挥发性有机化合物,其中24种化合物在所有样品中都被稳定检测并进行定量。在10份肺癌患者呼气样品中,正丁醇的浓度范围为3.71~25.99ng/L,3-羟基-2-丁酮的浓度范围为3.81~25.67ng/L,均明显高于对照组的浓度(P<0.05)。结论正丁醇和3-羟基-2-丁酮可能是鉴别早期肺癌有效的肿瘤标志物,固相微萃取-气相色谱/质谱法对于呼气中痕量挥发性有机化合物成分的成功定量分析为今后的肿瘤呼气诊断分析奠定了方法学基础。

Quantitative analysis of trace volatile organic compounds in the breath of early stage lung cancer patients

Abstract:Objective To quantitate the trace volatile organic compounds(VOCs) in breath of normal human and early-stage lung cancer patients with the methodology of gas chromatography(GC)/mass spectrometry(MS) coupled with solid phase microextraction(SPME).Methods The breath samples were collected from 10 normal human and 10 patients with stage lung cancer using Tedlar bags.The VOCs were extracted with SPME,analyzed by GC/MS and compared with the standards.Wilcoxon rank sum test was used for assessing the statistical significance.Results The quantity of VOCs detected in each breath sample ranged from 74 to 114.Among them 24 VOCs were determined quantitatively in all subjects.In the breath of patients the concentrations of 1-butanol and 3-hydroxy-2-butanone ranged from 3.71 ng/L to 25.99 ng/L,and from 3.81 ng/L to 25.67 ng/L,respectively,which were much higher than those in normal breath(P<0.05).Conclusion 1-Butanol and 3-hydroxy-2-butanone might be the useful tumor markers for discerning early stage lung cancer.The successful quantitative analysis of trace VOCs in human breath builds the methodological basis for further lung cancer diagnosis.
