冠心病患者血清可溶性纤维介素2的变化及其与炎症因子的关系

摘    要:目的:探讨冠心病患者外周血清中可溶性分泌型纤维介素2(sFGL2)水平及其与炎症因子的关系。方法选择52例初诊急性冠脉综合征(ACS)患者、50例初诊稳定性心绞痛(SAP)患者和50例冠脉造影正常者作为研究对象,分别测定外周血 CD4+ CD25+ CD127low 调节性 T 淋巴细胞(Treg)、外周血 sFGL2、炎症因子白细胞介素-10(IL-10)、转化生长因子-β1(TGF-β1)、肿瘤坏死因子-α(TNF-α)和超敏C 反应蛋白(hs-CRP)水平,RT-PCR 测定外周血淋巴细胞膜型纤维介素2(FGL2)mRNA 表达量,并进行统计学分析。结果与对照组和 SAP 组相比,ACS 组外周血 CD4+ CD25+CD127low Treg 百分比及血清 sFGL2、IL-10、TGF-β1显著降低(P <0.01),而 TNF-α和 hs-CRP 显著升高(P <0.01)。血清sFGL2与外周血 Treg、IL-10、TGF-β1呈正相关性,与 TNF-α和 hs-CRP 呈负相关性。逐步多元回归分析表明,sFGL2与外周血 Treg 和 IL-10呈正相关性。结论 ACS 患者外周血sFGL2明显降低,可以反映循环血 CD4+ CD25+ CD127low Treg数量及血清 IL-10水平,sFGL2降低可能参与了 ACS 发生和发展。

Changes of serum sFGL2 level and its relationship with inflammation cytokines in patients with coronary heart disease
Abstract:Objective To investigate the serum level of soluble fibrinogen-like protein 2(sFGL2)and its relation-ship with inflammation cytokines in the patients with coronary artery disease. Methods 152 consecutive patients were enrolled in this study. Patients were divided into acute coronary syndrome(ACS)group,stable angina pecto-ris(SAP)group and control group according to coronary angiography. Flow cytometry was employed to detect num-ber of circulating CD4 + CD25 + CD127low Treg cells. Serum concentrations of sFGL2 and inflammation cytokines in-cluding IL-10,TGF-β1,and hs-CRP were measured by ELISA. FGL2 mRNA in peripheral blood mononuclear cells (PBMCs)obtained from these patients was measured by RT-PCR. The association of sFGL2 with circulating CD4 +CD25 + CD127low Treg and inflammation cytokines was analyzed. Results Compared with the control and SAP group,the concentrations of sFGL2 were significantly lower(P < 0. 01),while concentrations of IL-10 and TGF-β1 significantly decreased(P < 0. 01). TNF-α and hs-CRP significantly elevated in ACS group(P < 0. 01). FGL2 mRNA measured by RT-PCR indicated that expression of FGL2 significantly reduced in ACS group compared with SAP and the control group. Serum concentration of sFGL2 was positively correlated with circulating CD4 + CD25 +CD127low Treg level,IL-10 and TGF-β1 while negatively correlated with TNF-α and hs-CRP. Stepwise multi-re-gression analysis showed that serum sFGL2 level was significantly associated with circulating CD4 + CD25 + CD127low Treg and IL-10 level. Conclusion Decreased concentration of sFGL2 is an independent risk factor for ACS. Changes in sFGL2 concentration can reflect the levels of circulating CD4 + CD25 + CD127low Treg and IL-10.
