超声心动图评价心外膜脂肪组织与2型糖尿病并发冠心病的相关性

摘    要:目的 利用超声心动图评价心外膜脂肪组织(EAT)厚度变化与2型糖尿病并发冠心病(CAD)的发生是否具有相关性.方法 筛选健康人群120例(对照组),经临床确诊单纯2型糖尿病25例(单纯2型糖尿病组)及糖尿病并发CAD患者24例(CAD组)作为研究对象,共计169例;男性86例,女性83例,平均(64.77±14.80)岁.利用超声心动图于左室长轴切面、心尖四腔心切面及剑下四腔心切面分别测量右室前壁、左室后壁、心尖部及右室游离壁心外膜脂肪组织厚度,对相同部位心外膜脂肪组织的厚度进行单因素方差分析及组间比较.结果 对照组、单纯2型糖尿病组及CAD组右室前壁各组EAT厚度比较,差异有统计学意义(P<0.05),其他观测部位EAT厚度各组间比较,差异均有显著统计学意义(P<0.01).CAD组各观测部位EAT厚度均较对照组及单纯2型糖尿病组明显增厚,差异有统计学意义(P<0.05);单纯2型糖尿病组与对照组各观测部位EAT厚度差异无统计学意义(P>0.05).结论 EAT的增厚与2型糖尿病CAD的发生存在正相关;超声心动图可以便捷、有效地评价EAT的增厚.

Evaluation of the Correlation Between Epicardial Adipose Tissue and Type 2 Diabetes Cardiovascular Complicating Coronary Heart Disease with Echocardiography

Abstract:Objective To evaluate the correlation between the thickness of epicardial adipose tissue(EAT) and type 2 diabetes complicating coronary heart disease(CAD) with echocardiography. Methods Screening healthy people as a control group 120 cases, and the clinical diagnosis of 25 cases of simply type 2 diabetes and complicating CAD in diabetic patients as research subjects 24 cases. A total of 169 cases (86 males,83 females,mean age 64.77! 14. 80 years) were enrolled in this study. The EAT thickness including right ventricular anterior wall, left ventricular wall, the apex part and right ventricular free wail was measured respectively in left ventricular long axis, apical four-chamber view, and the xiphoid four-chamber view with echocardiography. To analyze the data of the EAT thickness at the same parts by ANOVA and groups. Results There was significant statistical difference (P〈 0.05)at the right ventricular anterior wall and very significant statistical difference (P〈O. 01)at the other parts between the control group, the simply type 2 diabetes group and the group of CAD. The EAT thickness of group of CAD was significantly thickened than that of the control group and the simply type 2 diabetes group at different observing parts(P〈0.05). There was no significant difference between the simply type 2 diabetes group and the control groups about EAT thickness at parts of each observation (P 〉 0. 05) Conclusion There is a positive correlation between the EAT thickening and the occurrence of the type 2 diabetes complicating CAD. Echocardiography can evaluate the EAT thickening easily and effectively.
