关于新型农村合作医疗的几个伦理问题的分析

新型农村合作医疗不仅取决于自愿原则下农民的参与和支付能力,而且和保险制度的设计有关,更离不开效率、公平和健康的改进这些长远的目标。从筹资、保险制度设计方案、不同地区的制度可持续性三个方面的评价指标,结合社会公益原则、公平原则和效益合理原则进行伦理分析,对当前新型农村合作医疗实施的过程中诸如逆向选择、零收益现象等进行了讨论并提出了解决方法,指出构建基于伦理的分析与审视体系内的新型农村合作医疗制度评价体系的重要性。

Analysis of Ethical Problems about New Rural Cooperative Medicine

According to the ethical principle of voluntariness,survival and further development of new rural cooperative medical system relies on not only farmer's capacity of participation and payment,but also design of the related insurance system,and some long-term target such as the improvement in efficiency,equity and health care.Ethical analysis,which is based on the combination of social public-spirited principle,fair principle and reasonable performance principle,is carried on from fund raising system,design of insurance system,and sustainable development of systems in different regions.This article analyzes some issues in the current new rural cooperative medical system implement process,such as contrary choice,zero income phenomenon etc,and comes up with related countermeasures.By the above analysis,this paper concludes that setting up an evaluation system inside the new rural cooperative medical system is of great importance.
