心电门控的全胸部CTA在成人先天性心脏病中的诊断价值

目的:探讨心电门控的64层螺旋CT全胸部血管成像在成人先天性心脏病中的诊断价值。方法:搜集我院经手术证实的20例复杂性先天性心脏病患者,所有患者术前均行64层螺旋CT的全胸大血管检查,并和超声心动图结果对照。结果:以手术结果为金标准,64层螺旋CT和超声心动图在复杂性先天性心脏病方面的诊断准确率分别为92.30%、90.38%,尤其在诊断心外畸形方面,两者的准确率分别为96.15%、84.61%,CT要优于超声。结论:心电门控的64层MSCT(Multislice CT,MSCT)全胸大血管技术在成人CHD中有较高的诊断价值,不仅能检查出心内和心外大血管畸形,而且能判断有无胸部病变,对于术前评估有较高的应用价值。

Diagnostic value of electrocardiograph-gated chest CT angiography in adult congenital heart disease

Objective： To evaluate the value of electrocardiograph-gated spiral CT angiography for the thoracic aorta in adult with complex congenital heart disease（ CHD）. Methods： The data were collected in 20 patients with CHD,who received 64-slice spiral CT angiography of the thoracic aorta before operation,and compared with both surgical and echocardiography（ ECG） findings. Results： On the surgical findings as gold standard basis,64-slice CT and ECG led to accurate diagnosis rate by 92. 30% and 90. 38%,respectively,for the complex CHD,especially in extracardiac abnormalities,the accuracy of the previous two techniques was 96. 15% and 84. 61%,which was superior to ultrasonography. Conclusion： Electrocardiograph-gated 64-slice spiral CT has a higher diagnostic value in adult CHD,especially in detection of the extracardiac and intracardiac abnormalities as well as remaining thoracic lesions before operation.
