颈段高位食管内支架的介入治疗

目的探讨颈段高位食管内支架的个性化选择,食管入口位置的确定及术后的临床疗效。方法 22例颈段高位食管癌患者分别置放个性化支架,以梨状隐窝下极水平下2CM作为食管入口位置释放支架后,其支架上缘即在食管内。结果食管内支架置入全部成功,其中1例因放置食道支架后出现呼吸困难,立即再置入气管支架后呼吸困难缓解;2例同时行气管、食道支架置入术,患者呼吸状况改善,7例合并瘘的患者,1例1周后食道大出血死亡,3例术后1月因感染未能控制致呼吸衰竭死亡,3例3个月后恶液质多器官衰竭死亡;13例6个月后因肿瘤进展或转移死亡;2例患者经过综合治疗及对症处理1年后,支架尚通畅,能进食流质。结论颈段高位食管内支架置入,能有效解除狭窄或堵塞瘘口,提高患者生存质量。

Interventional therapy on upper esophageal

Objective To explore the personalized choice of high level esophageal stents,experience of finding start point of esophageal and clinical effects.Methods The esophageal stent was selected on the bases of the characteristics,length,sites of 22 patients with upper esophageal diseases,2cm under piriform recess as esophageal entrance point,the top of the stent was completely lied inside the esophageal.Results All stents were placed successfully,considerable satisfied results were received.Conclusion The stents placement of the stricture of cervical esophagus can lessen the patient＇s condition effectively and improve the survival quality of the patients.
