王斌谢俊敏式保宫U手术治疗弥漫性子宫腺肌病术后妊娠结局的分析

目的探讨王斌谢俊敏式保宫U手术治疗弥漫性子宫腺肌病的术后妊娠结局。方法回顾性分析2010年8月至2018年10月本院收治的268例有生育要求的弥漫性子宫腺肌病患者的临床资料,均行王斌谢俊敏式保宫U手术治疗,且手术后宫腔置入球囊防止宫腔塌陷。观察手术情况,术后不同时间血清激素水平与糖类抗原(CA125)水平、妊娠情况及新生儿结局。结果术后1、3、6、12个月,患者的CA125、黄体生成素(LH)、孕酮(P)、睾酮(T)、催乳激素(PRL)水平均呈降低趋势,雌二醇(E₂)、卵泡生成素(FSH)水平均呈升高趋势,差异有统计学意义(P<0.05);268例术后患者自然妊娠9例,试管婴儿妊娠25例,最后获得活婴32例,早产21例。结论弥漫性子宫腺肌病患者行王斌谢俊敏式保宫U手术治疗的效果较好,部分患者可以妊娠或辅助生殖妊娠。

Analysis of pregnancy outcome after WANG Bin,XIE Junmin' s uterine preservation U surgery in the treatment of diffuse adenomyosis

Objective To investigate the pregnancy outcome after WANG Bin,XIE Junmin's uterine preservation U surgery in the treatment of diffuse adenomyosis.Methods The clinical data of 268 patients with diffuse adenomyosis who had fertility requirements and were treated in our hos-pital from August 2010 to October 2018 were analyzed retrospectively.All patients were treated with WANG Bin,XIE Junmin's uterine preservation U surgery,and balloon was placed in the uterine cavity after operation to prevent uterine cavity collapse.The operation conditions,serum hormone levels and carbohydrate antigen(CA125)levels at different times after operation,pregnancy and neonatal outcomes were observed.Results At 1,3,6 and 12 months after operation,the levels of CA125,luteinizing hormone(LH),progesterone(P),testosterone(T)and prolactin(PRL)of patients were decreased,while the levels of estradiol(E₂)and follicle stimulating hormone(FSH)were increased,the difference was statistically significant(P<0.05);of 268 patients,9 cases had natural pregnancy,25 cases had IVF pregnancy,and 32 cases of live infants and 21 cases of preterm births were ob-tained.Conclusion The effect of WANG Bin,XIE Junmin's uterine preservation U surgery in patients with diffuse adenomyosis is better,some pa-tients can have pregnancy or assisted reproductive pregnancy.
