TEE在卵圆孔未闭诊断及介入封堵术中的应用研究

目的探究经食道超声心动图(TEE)在卵圆孔未闭诊断及介入封堵术中的应用效果。方法选取2017年8月至2019年12月本院接收的40例疑似卵圆孔未闭患者为研究对象,术前患者均行TEE检查、经胸超声心动图(TTE)检查,在确诊后使用TEE引导进行介入封堵术,观察记录TEE、TTE检查结果以及TEE引导介入封堵术的术后疗效。结果40例患者中,TEE诊断出30例,TTE诊断出15例,TEE检出率明显高于TTE(P<0.05);30例卵圆孔未闭中均有明显偏头痛或脑卒中病史,仅15例患者接受封介入封堵术,患者术后3个月的头痛影响测定(HIT-6)评分低于术前,差异有统计学意义(P<0.05)。结论TEE能有效诊断出卵圆孔未闭,检出率高于TTE,能为手术提供可靠的参考数据,确保介入封堵术顺利完成。

Application of TEE in the diagnosis and interventional closure of patent foramen ovale

Objective To explore the effect of transesophageal echocardiography(TEE)in the diagnosis of patent foramen ovale and interventional closure.Methods 40 patients with suspected patent foramen ovale admitted to our hospital from August 2017 to December 2019 were selected as the study subjects.All the patients received TEE,transthoracic echocardiography(TTE)and TEE guided interventional closure after diagnosis.The results of TEE,TTE and TEE guided interventional closure were observed and recorded.Results Among the 40 patients,TEE was diagnosed in 30 cases,TTE in 15 cases,TEE was detected significantly higher than TTE.Among the 30 cases diagnosed with patent foramen ovale,all had history of significant migraine or stroke,and only 15 patients received closure and interventional blockade.The headache impact test(HIT-6)score of the patients 3 months after the operation was lower than that before the operation,and the difference was statistically significant(P<0.05).Conclusion TEE can effectively diagnose patent foramen ovale with higher detection rate than TTE,and can provide reliable reference data for surgery to ensure the successful completion of interventional closure.
