子宫动脉栓塞／化疗栓塞术在治疗剖宫产后子宫切口妊娠中的应用价值

目的探讨子宫动脉栓塞／化疗栓塞术在剖宫产后子宫切口妊娠清宫术前后的应用价值。方法对3例剖宫产后子宫切口妊娠因清宫术中大出血急诊行双侧子宫动脉栓塞术；对1例剖宫产后子宫切口妊娠杀胚治疗失败，2例剖宫产后子宫切口妊娠行双侧子宫动脉化疗栓塞术。6例均在介入治疗后1周内在B超引导下行清宫术。结果6例均成功施行双侧子宫动脉栓塞／化疗栓塞术，1周内顺利行清宫术，术中出血量少。结论双侧子宫动脉栓塞／化疗栓塞术不仅可以为剖宫产后子宫切口妊娠的保守治疗提供安全保障，而且对剖宫产后子宫切口妊娠清宫术中大出血的患者可以达到即时止血的目的，避免患者子宫被切除的严重后果，是一种微创、安全、有效、并发症少的治疗方法。

Valuation of uterine artery embolization/chemoembolization in patients with cesarean scar pregnancy

Objective To explore the valuation of uterine artery embolization/chemoembolization before/after uterine curettage in patients with CSP.Material and methods Collecting the clinical materials of 6 patients with CSP who had interventional therapy. Bilateral uterine artery embolization was performed emergently in 3 patients who initially experienced failed uterine curettage lead to severe bleeding. Bilateral uterine artery chemoembolization was performed in 3 patients, One due to the failure of embryo-killing treatment with methotrexate,Two as soon as be diagnosed CSP.Curettage was performed under sonography guidance within a week after interventional therapy.Results Interventional treatments were performed successfully in 6 patients followed by uterine curettage within a week with only a little hemorrage.Surgical treatment and blood transfusion were not required. Fertility was preserved.Conclusion Uterine curettage followed uterine artery embolization/chemoembolization can be used as the primary treatment for CSP. It is a minimally-invasive safe efficacious and less complications technique.
