左-卡尼汀对终末期肾病患者微炎症和氧化应激状态的影响

摘    要:目的探讨左-卡尼汀对终末期肾病(ESRD)患者微炎症和氧化应激状态的影响。方法选择住院及长期门诊治疗的ESRD患者62例,分为血液透析组、保守治疗组,二组又各自分为左-卡尼汀组和对照组两个亚组,另设15例体检正常者为健康对照组,左-卡尼汀组给予静脉注射左-卡尼汀1.0g,3次/周,进行为期6月的随访,分别于人选初、3、6月后测定微炎症指标:高敏C反应蛋白(hs—CRP)、白介素-6(IL-6);氧化应激指标:丙二醛(MDA)、血浆谷胱甘肽过氧化物酶(GSH—Px)。结果血液透析组、保守治疗组患者hs—CRP、IL-6和MDA水平高于健康对照组,GSH—Px低于健康对照组(P〈0.01);血液透析组患者hs—CRP水平高于保守治疗组患者,GSH—Px低于保守治疗组患者(P〈0.05);左-卡尼汀治疗后与治疗前比较hs,CRP、IL-6和MDA水平降低,GSH.Px水平升高fP〈0.05)。结论ESRD患者均存在一定程度的微炎症及氧化应激状态增强,左-卡尼汀可明显改善微炎症及氧化应激状态。

Abstract:Objective To investigate the effects of levo-carnitine on microinflammation and oxidative stress status in patients with end-stage renal disease (ESRD). Methods 62 outpatients and inpatients with ESRD in our hospital were selected and divided into two groups of hemodialysis group and conservative treatment group. 15 healthy volunteers for physical examination served as control group. Two groups were randomly divided into levo-carnitine therapy group and control group. The levo-carnitine therapy group was injected levo-carnitine (1.0 g) 3 times weekly for 6 months. The serum high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), maleic dialdehyde (MDA), glutathione peroxidase (GSH-Px) were measured as the indicators of microinflammation and oxidative stress status in these patients at the beginning, 3 months later and 6 months later respectively. Results Levels of hs-CRP, IL-6, MDA in hemodialysis group and conservative treatment group were significantly higher than those in the healthy control group, while the serum GSH-Px activity was lower. Compared with the pre-dialysis patients, hemodialysis patients exhibited higher level of hs-CRP, and lower level of GSH-Px activity. Compared with the indicators before levo-carnitine therapy and after levo-carnitine therapy, the after levo-carnitine therapy exhibited lower levels of hs-CRP, IL-6, and MDA, and higher level of GSH-Px activity. Conclusion The patients with ESRD have different degrees of increase in microinflammation and oxidative stress status. Levo-carnitine can improve the state of microinflammation and oxidative stress in patients with ESRD.
