家庭式分娩模式对促进自然分娩的影响

目的 观察产科家庭式分娩模式对分娩方式、产程时间、产后出血、新生儿窒息、产妇及家属对护理工作的满意度等的影响.方法 随机抽取产科实施家庭式分娩模式的孕产妇100例作为观察组,对照组选择同期入住传统产房分娩接受常规基础护理和产科专科护理的孕产妇100例.比较两组产妇分娩情况和产妇及家属对护理工作的满意度等方面的差异.结果 观察组剖宫产10例,自然分娩90例,剖宫产率为10.0%,对照组剖宫产25例,自然分娩75例,剖宫率为25.0%,观察组剖宫产率明显低于对照组（P＜0.01）.观察组自然分娩产妇的第一、二产程及总产程的时间短于对照组,差异有统计学意义（P＜0.01）,但两组第三产程时间比较差异无统计学意义（P＞0.05）.观察组产后出血发生率与对照组比较差异无统计学意义（P＞0.05）,而新生儿窒息发生率显著低于对照组（P＜0.01）.观察组产妇及家属对护理工作的满意度显著高于对照组（P＜0.01）.结论 实施家庭式分娩模式可降低剖宫产率、缩短自然分娩产程时间、减少产后并发症的发生,提高产科整体护理质量.

Effect of family delivery mode on natural birth

Objective To observe the influence of family delivery mode on delivery way, duration of delivery, postpartum hemorrhage, neonatal suffocation and puerpera and their families＇ satisfaction degree for nursing. Methods 100 puerpera who chose the family delivery mode were selected as the observation group. At the same time, 100 pu- erpera who deliveried in traditional delivery room and received conventional nursing and specialist care as the control group.Then,differences of delivery situation of puerpera and their parents＇ satisfaction degree for nursing were com- pared between two groups.Results In the observation group, 10 cases got cesarean delivery（cesarean delivery rate was 10.0%） and the rest 90 cases got natural delivery. 25 cases in the control group got cesarean delivery（cesarean delivery rate was 25 %） and 75 cases got natural delivery.Cesarean delivery rate of the observation group was significantly lower than that of the control group（ P 〈0.01）. The first and second stage of labor as well as total stage of labor in the observation group were all shorter than those of the control group（ P 〈0.01） ,so the difference was statistically significant（ P %0.01）. However, there was no significant difference in the third stage of labor between groups（ P 0.05）.There was no significant difference in the comparison of incidence of postpartum hemorrhage in both groups（ P ~0.05）. The rate of neonatal suffocation of the observation group was lower than that of the control group（ P 〈0.01）.Nursing satisfaction of puerpera and their families were better in the observation group（ P 〈0.01）.Conclusion Family delivery mode can shorten the duration of delivery, reduce the rate of cesarean section and complications.lt can also improve the quality of holistic nursing care of obstetric department.
