三种不同方法在治疗慢性肥厚性鼻炎的疗效比较

目的比较三种下鼻甲手术方式的疗效,选择适当的治疗慢性肥厚性鼻炎的方法。方法将183例患者分为三组,A组（57例）行下鼻甲部分切除术,B组（65例）行下鼻甲黏膜下微波治疗术,C组（61例）行下鼻甲黏膜下低温等离子消融术,观察三组术后6个月时手术疗效,记录三组术中出血量及创面愈合时间,术后鼻干、鼻出血、嗅觉减退等并发症。结果三组疗效比较差异无统计学意义（P〉0.05）;术中出血量、创面愈合时间A组高于B、C组（P〈0.05）;术后出血A组高于B、C组（P〈0.05）,术后鼻腔干燥和嗅觉减退发生率B组高于A、C组,C组并发症最少（P〈0.05）。结论低温等离子射频消融术治疗慢性肥厚性鼻炎疗效确切,并且保护了鼻黏膜的完整性,并发症少,可优先采用。

Three different methods in treating chronic hypertrophic rhinitis

Objective To compare the three methods of inferior turbinate surgery,and to choose the appropriate treatment method of chronic hypertrophic rhinitis.Methods 183 patients were randomly divided into three groups： group A（n=57） were treated by partial inferior turbinectomy,group B（n=65） were treated by inferior turbinate mucosa microwave therapy and group C（n=61） were treated by low-temperature plasma ablation on inferior turbinate.The effects in the three groups were observed for 6 months after operation.Blood loss in operation and wound healing time of three groups were recorded.Complications such as nasal dryness,epistaxis and hyposmia were recorded in three groups.Results There was no significant difference among the effect of the three groups（P〉0.05）.Blood loss,wound healing time in group A were higher than that in group B and group C（P〈0.05）.There was no significant difference in blood loss,wound healing time between group B and C.Postoperative blood loss in group A were higher than that in group B and group C（P〈0.05）.The incidence of postoperative nasal dryness and hyposmia in group B were higher than that in group A and group C,and complications in group C is the least in the three groups（P 〈0.05）.Conclusion The low-temperature radiofrequency ablation treatment of chronic hypertrophic rhinitis is effective method and with fewer complications.
