高脂血症患者血管内皮功能的超声检测分析

目的探讨高脂血症对血管内皮功能的影响。方法应用高频超声检测65例高脂血症患者其中高甘油三酯（TG）组25例，高胆固醇（TC）组16例，混合性高脂血症组24例]的肱动脉血管内皮依赖性舒张功能和内皮非依赖性舒张功能并与20例血脂正常的对照组相比较。结果高1℃组、高TC组、混合性高脂血症组血流介导的肱动脉内径变化的百分率依次为（5．10±1．69）％、（5．17±1．90）％、（3．40±1．10）％]以及峰值流速变化百分率依次为（46．51±7．65）％、（53．44±9．90）％、（28．57±4．82）％]明显低于对照组（13．52±3．48％和66．26±11．28％）（P〈0．05），其中以混合性高脂血症组变化最显著，高TG组和高TC组之间差异没有显著性（P〉0．05）。各组肱动脉内径的基础值分别为（3．73±0．77）mm、（3．78±0．53）mm、（4．12±0．75）mm和（3．92±0．75）mm]及含服硝酸甘油后肱动脉内径变化的百分率分别为（16．82±4．61）％、（19．63±6．82）％、（18．20±5．31）％和（17．20±2．60）％]，差异无显著性（P〉0．05）。结论各型高脂血症患者血管内皮依赖性舒张功能较正常人明显受损，内皮非依赖性舒张功能无改变。

Evaluation of the artery endothelium function in patients with hyperlipidemia using high -resolution ultrasound

Objective To study the effect of hyperlipidemia on the endothelium function.Methods 65 patients with hyperlipidemia(25 cases with hypertriacylglycerolemia,16 cases with hypercholesterolemia,24 cases with mixed hyperlipidemia) were included in this study.To measure the endothelium-dependent relaxing function and the endoth-elium-independent relaxing function of brachial artery with high resolution ultrasound and to compare with that in 20 cases with normal lipidemia in control group.Results The flow-mediated dilatation and peak flow velocity in hypertriacylglycerolemia group,hypercholesterolemia group and mixed hyperlipidemia group were significantly lower than those in control group(P<0.05).There was no significant difference between hypertriacylglycerolemia group and hypercholesterolemia group.There was no significant difference between three hyperlipidemia groups and control group in baseline of brachial artery and the nitroglycerin-induced dilatation.Conclusion The endothelialdependent relaxing function are significantly impaired in all kinds of patients with hyperlipidemia,but the endothelial-independent relaxing function are normal.
