胫骨骨折三种治疗方法的疗效评估

目的　通过对临床上最常用的三种治疗胫骨骨折疗效的评估 ,确定哪一种治疗方法更合理化。方法自 1994～ 1998年采用单侧多功能外固定支架 ,自动加压钢板和带锁髓内钉治疗各种新鲜胫骨骨折 5 82例。结果经术后平均 1年 8个月的随访 ,三种方法治疗胫骨骨折 ,骨折愈合的时间 :①单侧多功能外固定支架愈合稍快平均为16 .2周 ;②带锁髓内钉愈合时间平均为 16 .4周 ;③自动加压钢板愈合时间平均为 2 0 .6周。结论　①单侧多功能外固定支架治疗各类新鲜胫骨骨折的疗效稍优于其它两种内固定 (自动加压钢板和带锁髓内钉 ) ;②手术操作简单 ,适应症广 ;③并发症少 ,无需二次手术取内固定物 ;④适合基层医院推广使用。

The clinical evaluation of three methods for management of tibia fracture

Objectives To study which was more advisable by evaluating three methods for the treatment of tibia fracture.Methods 582 cases of freshtibia fracture were treated with unilateral mutifunetional excernal finator dynamic compressive plate and interlocking intermedullry nail.Results All the patients were followed up for an average of 20 months.The union and the evidence of camplication were different among there methods,the average period of union was 16.2 weeks in unilateral excernal fixator 16.4 weeks in interlocking nail 20.6 weeks in DCP.Conclusions The dinical effect of excernal fixator was better than dynamic compressive plate (DCP) and interlocking nail for the treatment of tibia fracture.It indicated the advantage of convenienle in the procedure the less complication and the spreading in grass-roots hospital.
