PCR—SSCP快速检测脊柱结核耐药基因

摘    要:目的 了解临床脊柱结核耐药情况 ,探讨耐药基因PCR-SSCP对临床的应用价值.方法 43例脊柱结核患者结核肉芽组织应用BacT/ ALERT 3D结核分支杆菌快速培养系统培养,所得临床分离株行药敏试验,对耐药基因katG、r p s L行PCR-SSCP.结果 43例结核肉芽组织标本培养后19例阳性,其中6株存在不同程度耐药 ,总耐药率为31%.耐异烟肼株 katG突变率为 10% ,耐链霉素株 rpsL突变率为21% , 耐药基因检测平均用时为14h.结论 PCR-SSCP可快速检测脊柱结核耐药基因,与常规药敏试验结合可更能准确地反应脊柱结核患者的耐药情况.

The rapid detection of spinal tuberculosis drug resistance genes by PCR-SSCP

Abstract:Objective To understand the drug - resistant situation of clinical spinal tuberculosis, and to explore PCR - SSCP clinical application value of coagulase analysis of drug - resistant genes. Methods BacT / ALERT 3D mycobacterium tuberculosis fast training system was used for 43 patients with spinal tuberculosis, and patients with tuberculosis granulomatous application, and cultivated clinical separation plant susceptibility test line, and katG drug - resistant genes, r p s L were analyzed by PCR - SSCP. Results The tuberculosis granulation tissue samples of 43 cases with spinal tuberculosis were trained, 19 out of 43 cases were positive; there were 6 cases with drug resistance in different degree. The total drug resistance was 31%. KatG isoniazid strains mutation rate was 10%. Strep- tomycin resistance strain r p s L mutation rate was 21%. Resistance gene detection took 14 hours on average. Conclusion PCR - SSCP can rapidly detect the spinal tuberculosis drug - resistance genes. And resistant gene combination of drug sensitive test can be more accurately response spinal tuberculosis patients of drug resistance.
