清喉利咽颗粒联合金嗓散结丸防治支撑喉镜下CO2激光治疗声带息肉后发声障碍的临床效果

目的 分析清喉利咽颗粒联合金嗓散结丸防治支撑喉镜下CO₂激光治疗声带息肉后发声障碍的临床效果。方法 选取宝鸡市中医医院2018年7月至2020年7月收治的声带息肉患者88例作为研究对象,按照随机数字表法分为观察组和对照组,各44例。患者均给予支撑喉镜下CO₂激光治疗,术后对照组给予常规治疗,观察组在对照组基础上给予清喉利咽颗粒联合金嗓散结丸,比较两组的临床治疗效果。结果 治疗后,观察组总有效率明显高于对照组（P<0.05）;观察组声音嘶哑、咽干灼热疼痛、咽喉异物感、咽痒咳嗽等临床症状消失时间均明显短于对照组（P<0.05）;两组患者基频微扰,振幅微扰,噪谐比,标准化噪声能量等嗓音功能均明显低于治疗前,且观察组低于对照组（P<0.05）。治疗后,两组的嗓音障碍指数量表（voice handicap index,VHI）评分（包括功能,生理,情感,总分）与嗓音嘶哑评估GRBAS评分（grade roughness breathiness asthenia strain,GRBAS score）中的各项评分均明显低于治疗前,且观察组低于对照组（P<0.05）。结论 清喉利咽颗粒联合金嗓散结丸防治支撑喉镜下CO₂激光治疗声带息肉,可有效地改善患者的临床症状与发声障碍,提高生活质量。

Clinical effect of Qinghou Liyan Granules combined with Jinsang Sanjie Pill on prevention and treatment of vocal cord polyp dysphonation by CO2 laser under self-supporting laryngoscope

Objective To analyze the clinical effect of Qinghou Liyan Granule combined with Jinsang Sanjie Pill in preventing and treating vocal cord polyp with CO₂ laser under self-supporting laryngoscope.Methods 88 patients with vocal polyp in Baoji Traditional Chinese Medicine Hospital were selected as the research objects, and divided into observation group and control group with 44 cases in each group, according to random number table. All patients were treated with CO₂ laser under self-retaining laryngoscope, and the control group was given routine treatment after operation, while the observation group was given Qinghou Liyan Granule combined with Jinsang Sanjie Pill on the basis of the control group, and the clinical therapeutic effects of the two groups were compared.Results After treatment, the total effective rate of clinical treatment in the observation group was significantly higher than that in the control group （P<0.05）. The disappearance time of symptoms such as hoarseness, sore throat, foreign body sensation in throat, itchy throat and cough in observation group were significantly shorter than those in control group （P<0.05）. The voice functions of basic frequency perturbation, amplitude perturbation, noise-harmonic ratio, standardized noise energy in the two groups were significantly lower than those before treatment, and those in the observation group were lower than the control group （P<0.05）. After treatment, the voice handicap index （VHI） score （including function, physiology, emotion, total score） and the grade, roughness, breathiness, asthenia, strain （GRBAS） score of the two groups were significantly lower than those before treatment, and those in the observation group were lower than those in the control group （P<0.05）.Conclusion Qinghou Liyan Granule combined with Jinsang Sanjie Pill has a significant clinical effect in preventing and treating vocal cord polyp with CO₂ laser under self-supporting laryngoscope, which can effectively improve the clinical symptoms and dysphonia of patients and improve the quality of life.
