腹腔镜胃十二指肠溃疡穿孔修补术30例

目的探讨腹腔镜胃十二指肠溃疡穿孔修补术的疗效及优越性。方法回顾性分析30例腹腔镜胃十二指肠溃疡穿孔修补术,以同期26例胃十二指肠溃疡穿孔患者行开腹作为对照组。比较两组手术时间、术中出血量、肠蠕动恢复时间、术后住院时间及并发症的发生情况。结果腹腔镜组与开腹组手术时间分别为68±11 min和102±21 min（P〈0.05）;术中出血量分别为36±6 mL和90±12 mL（P〈0.01）;肠蠕动恢复时间分别为21±3 h和37±6 h（P〈0.05）;术后住院时间分别为5.5±1.2天和9.3±2.1天（P〈0.05）;术后并发症分别为1、6例。结论腹腔镜组具有创伤少、手术时间短、出血少、恢复快、住院时间短、并发症少等明显优势,具有临床应用价值。

The Clinical Analysis of Laparoscopic Repair of Gastro Duodenal Ulcer Perforation

Objective To research the curative effect and advantage of laparoscopic repair of gastroduodenal ulcer perforation. Method 30 cases of laparoscopic repair of gastroduodenal ulcer perforation were retrospectively reviewed with 26 cases of gastroduodenal ulcer perforation open in the same period as the control. The operative time, intraoperative blood loss, postoperative recovery time of bowel function, postoperative body temperature and hospitalization of the two groups were compared respectively. Result The operative time of group of laparoscopy and open procedures was 68 ± 11 min, 102 ± 21 min （ P 〈 0.05 ） ; the intraoperative blood loss was 36 ± 6 mL, 90 ± 12 mL, （P 〈 0.01 ） ; the postoperative recovery time of bowel function was 21 ±3 h, 37 ± 6 h, （ P 〈0.05 ） ; hospitalization was were 5.5 ± 1.2 d, 9.3 ± 2.1 d （ P 〈 0.05 ） ; the pest - operative comphcations were 1, 6 cases respectively. Conclusion Laparoscopic repair of gastroduodenal ulcer perforation is safe as open procedures. The laparoscopic operation for gastroduodenal ulcer perforation has advantages such as minimal invasion, short operative time, less intraoperative blood loss, rapid recovery and shorter hospitalization time and less post - operative complications with clinical value.
