造釉细胞瘤的 X 线表现——90例分析

本文分析90例颌骨釉细胞瘤,均经病理证实,显示 X 线表现有不同的类型,即多房型;单房型;蜂窝型;混合型;小梁型及边缘型。特别提出了含牙单房造釉细胞瘤与含牙囊肿的鉴别点。并提出造釉细胞瘤病变区内所出现的不同致密影像,有不同的病理基础。

ROENTGENOLOGIC STUDY OF AMELOBLASTOMA AN ANALYSIS OF 90 CASES

Ameloblastoma is a kind of dental epithelial tumor of the jaw.An analysis of90 pathologically confirmed cases was made.The roentgenologic appearance ofthese tumors is variable.Among these cases,the multilocular type is the commonestone(39 cases,43.3%),others are unilocular type(30 cases,33.3%);honey-combedtype(9 cases,10%);mixed type(6 cases,6.7%);trabeculated type(4 eases,4.4%);and marginal type(2 cases,2.2%).It must be pointed out that the differentialdiagnosis between the unilocular ameloblastoma and the cyst is rather difficult,especially the unilocular ameloblastoma with tooth inside,thus its appearance isquite similar to that of a dentigerous cyst.Only 4 cases of tooth containing unilocular ameloblastoma are seen in thisseries,and are all located in the angle of the mandible with a tooth in the bottomof the cavity.Close observation of the films reveals that the characteristic changesare as follows:(1)The peripheral margin of the cystic cavity is nearly alwayslobulated,but often blurred in the adjacent part of the alveolus and with honey-combed appearance;(2)The neighboring teeth often show disappearance of thelamina dura and absorption of the tip.The dental root may also be cut off;(3)Onan average the age incidence is about thirty years old in unilocular ameloblastoma,and older than that of the dentigerous cyst.Roentgenologically some areas of the lesion show increase in density,but withdifferent appearance,they are correlated with pathology.
