肩胛盂倾斜角和扭转角与冈上肌腱全层撕裂的相关性分析

摘    要:目的 明确肩胛盂倾斜角和扭转角与冈上肌腱全层撕裂的关系.方法 42例经临床确诊为单侧冈上肌腱全层撕裂而对侧无任何临床症状的患者,用16排螺旋CT扫描,三维CT多平面重组及容积再现技术,测量双侧肩胛盂倾斜角和扭转角,运用自身配对t检验对比分析.结果 肩胛盂倾斜角:患侧为(97.6±4.0)°,健侧为(96.1±2.9)°,双侧具差异有统计学意义(P=0.001),患侧比健侧大;扭转角:患侧为(-3.2±5.0)°,健侧为(-3.1±4.7)°,双侧差异无统计学意义(P=0.79).结论 冈上肌腱全层撕裂与肩胛盂倾斜角大小具有相关性,与肩胛盂扭转角大小无关,提示肩胛盂倾斜角过大可能更容易引起冈上肌腱全层撕裂.

Abstract:Objective To investigate the relationship between glenoid inclination or version and supraspinatus tendon full-thickness tears. Methods Forty-two cases of unilateral supraspinatus tendon fullthickness tears were confirmed by clinical examinations and MRI scans. The bilateral glenoid inclination and version angles, measured by the volume rendering technique (VRT) and multi-planar reformation (MPR) of multi-slice computed tomography (MSCT) under 16-slice spiral CT scans, were compared by paired t test in all cases. Results The average inclination angle was (97.6 ± 4. 0) ° and (96. 1 ± 2. 9) at affected and healthy sides respectively. And the difference had statistical significance(P = 0. 001); the average version angle was (- 3. 2 ± 5.0) ° and (- 3. 1 ± 4. 7) ° at affected and healthy sides respectively. And the difference had no statistical significance (P = 0. 79). Conclusion The glenoid inclination angles are correlated with supraspinatus tendon full thickness tears while but the glenoid version angles are not. A larger glenoid inclination angle may be a positive predictive factor of supraspinatus tendon full-thickness tears.
