幽门螺杆菌对2型糖尿病患者血糖波动的影响

目的 探讨幽门螺杆菌(Hp)对2型糖尿病患者血糖波动的影响.方法 回顾性选择130例2型糖尿病患者,其中伴Hp阳件者72例、Hp阴性者58例,收集患者临床资料,包括血压、体重指数、胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、糖化血红蛋白、胰岛素抵抗指数及血糖水平.利用血糖仪下载的3 d全天7个时间点血糖谱总计21个血糖值输入强生血糖数据分析软件(OTDMS)系统进行分析,并做统计学处理来评估血糖波动.采用平均血糖水平的标准差(SDBG)、平均血糖波动幅度(MAGE)、最大血糖波动幅度(LACE)、低血糖例数作为本研究血糖波动评估参数.结果 两组患者的年龄、病程、血压、体重指数、血脂、糖化血红蛋白、胰岛素抵抗指数比较,差异均无统计学意义(均P>0.05).2型糖尿病Hp阳性组的平均血糖(MBG)、SDBG、MAGE、LAGE均高于糖尿病Hp阴性组(mmol/L:9.0±1.1比7.6±0.5,3.3±1.1比1.7±0.5,6.7±4.5比3.0±1.1,8.6±3.8比4.2±1.5,均P<0.05),低血糖例数百分比也高于糖尿病Hp阴性组16.6%(12/72)比5.1%(3/58),P<0.05].结论 Hp感染对于2型糖尿病患者血糖波动有明显影响,根除Hp有益于控制血糖、减少低血糖事件、保证血糖平稳达标.

Effects of Helicobacter pylori on blood glucose fluctuation in type 2 diabetic patients

Objective To investigate the effects of Helicobacter priori(Hp)on blood glucose fluctuation in type 2 diabetic mellitus.Methods 130 type 2 diabetic patients were selected,72 patients with Hp infection and 58 without Hp infection.Relevant clinical data:blood pressure,body mass index,cholesterol,triglyceride,low density lipoprotein cholesterol,high density lipoprotein cholesterol,glycosylated hemoglobin,insulin resistance index,liver function,and renal function were collected.The data of blood glucose levels at 7 time points,6-7 am,9-10 am,11-12 am,14-15 pm,17-18 pm,19-20 pm,and 22-23 pm in 3 days,totally 21 data,were collected.And input into the OTDMS data analysis software to evaluate the blood glucose fluctuation indexes:standard deviation of meoJi blood glucose (SDBG),mean amplitude of glycemic excursions(MAGE),large amplitude of glycemic excursions (IAGE).Results There were no significant differences in age,course of disease,blood pressure,body mass index,cholesterol,triglyceride,low density lipoprotein cholesterol,glycosylated hemoglobin,and insulin resistance index between these 2 groups(all P>0.05).The levels of MBG,SDBG,MAGE,LAGE,(mmol/L:9.0±1.1 vs 7.6±0.5,3.3±1.1 vs 1.7±0.5,6.7±4.5 vs 3.0±1.1,8.6±3.8 vs 4.2±1.5,all P<0.05)and incidence of hypoglycemia16.6%(12/72)vs 5.1%(3/58),P<0.05]were all higller in the Hp positive group than in the Hp negative group.Conclusion Hp infection has a significant effect on the daily blood glucose level and blood glucose fluctuation in the patients with type 2 diabetes.
