超声严重指数对急性胰腺炎分级诊断的平均效度评价

目的：对超声严重指数对急性胰腺炎诊断的平均效价进行探讨。方法：将2009年6月至2013年1期间就诊于我院的80例急性胰腺炎患者作为研究对象，所有研究对象均经螺旋CT增强扫描检查和超声造影检查。按照Balthazr 评分系统，对两种检查结果进行分级诊断和坏死程度诊断，对CT严重指数和超声严重指数进行比较。探讨超声严重指数在急性胰腺炎分级诊断中的平均效价。结果：两种诊断方法在炎症分级诊断的一致率PO为0．863，kappa值为0．812，X2＝21．47，P＜0．05。将两种检查手段对坏死灶诊断结果进行比较，其PO值为0．821，kappa 值为0．824，X2＝31．47，P＜0．05。对两种检查手段严重指数评分进行比较，其PO值为0．923，kappa值为0．878，X2＝34．23，P＜0．05。结论：超声造影严重指数能够对急性胰腺进行较好的分级诊断，可以准确的判断胰腺坏死灶范围，正确的评估病情。

Average Validity of Ultrasound Severity Index in the Evaluation of Acute Pancreatitis Classification

Objective：To investigate the average validity of ultrasound severity index in the evaluation of acute pancreatitis classification.Method：80 acute pancreatitis patients were chosen as research subjects, they all underwent enhanced spiral CT scan and ultrasound imaging .Accordance to the Balthazr scoring sys-tem, The severity index of CT and ultrasound, the necrosis and classification diagnosis were compared.Re-sults：In the diagnosis of inflammation grade of two methods, the concordance rate PO was 0.863, kappa value was 0.812, X2=31.47,P〈0.05.In the comparison of necrosis diagnostic results of the two methods, the PO value was 0.821, kappa value was 0.824, X2=31.47, P 〈0.05.In the comparison of severity in-dex score of the two methods, the PO value was 0.923, kappa value was 0.878, X2=34.23, P 〈0.05. Conclusion：Ultrasound imaging severity index can accurately determine the grades of acute pancreatic , it can determine the scope of pancreatic necrosis , accurate assessment of the condition .
