食管癌贲门癌经胸切除术后严重并发症27例临床分析

目的：探讨食管癌、贲门癌经胸切除术后严重并发症发生的主要原因及其防治措施。方法：回顾分析1988年8月至2001年8月在我院经胸切除的食管癌、贲门癌784例患者，其中食管癌569例，贲门癌215例。结果：术后发生吻合口瘘17例，死亡4例，乳糜胸10例，死亡2例。结论：食管吻合技术的提高、吻合方法的改进和围手术期的处理，使食管癌、贲门癌经胸切除术后严重并发症的发生率明显降低。

Clinical analysis of severe complications following surgical resection with thoracotomy in 27 patients with esophageal or cardiac carcinoma

Objective: To investigate the main reasons, care methods and preventing managements of severe complications following surgical resection with thoracotomy in esophageal or cardiac cancer patients . Methods: We retrospectively analyzed 784 patients with esophageal or cardiac cancer who were undertaken surgical treatments with thoracotomy in my hospital from. August 1988 to August 2001.569 cases are esophageal carcinoma , while the rest 215 cases are cardiac carcinoma. Results: Complications occurred postoperatively are including, anastomotic fistala in 17 cases,of which 4 patients died; and chylothorax in 10 cases,4 which 2 patients died. Conclusion: Improvements in the technology of esophago-gastrostomy and perioperative managements have significant importance in reducing the occurrence of complications following surgical treatment with thoracotomy in patients with esophageal or cardiac cancer.
