257例心肺复苏影响因素分析

摘    要:目的:了解3年来心肺复苏(CPR)现状,分析其影响因素,旨在提高心肺复苏成功率。方法:回顾性分析2005年1月至2008年1月我院急救中心复苏的257例院前及院内心搏骤停(CAcardiac arrest)患者,比较自主循环恢复(ROSC)成功组和失败组CPR开始时间、发病地点、病因、肾上腺素用量等。结果:全部病例ROSC成功组占8.2%,两组CPR开始时间(从心跳骤停至CPR开始时间)、发病地点、病因比较差异有统计学意义(P〈0.05),在肾上腺素用量、性别比较无明显差异。CPR成功率较低与心肺复苏开始时间、发病地点及复苏方法密切相关。结论:CA患者成功率较低,与"生命连"未彻底落实及急救水平低有关,普及全民急救知识,加强完善急救医疗体系建设,是提高CPR成功率的关键措施。

Abstract:Objective: To study the status of 257 cardiac arrest ( CA ) patients from January 2005 to January 2008 in our hospital and analyze its influencing factors in order to improve success rate of cardiopulmonary resuscitation ( CPR ). Method: A retrospective study was carried out in 257 CA Pre -hospital and inhospital patients. According as the restoration of spontaneous circulation ( ROSC ) was successful or not, the patients were divided into 2 groups. Many data such as the starting time of CPR,cause of diseases, space of fall ill, adrenaline dose, were compared. Result: The total success rate of ROSC was 8.2%. There was significant difference in the starting time of CPR,space of fall ill,cause of diseases. There's no significant difference in adrenaline dose and sex . Conclusion: Success rate of the CPR is poor because the "survival chain" does not be carried out completely, and at the same time, we can not overlook the facts of our poor emergency level. The key way to improve success rate fect the emergency medical service system. of CPR is to popularize emergency knowledge and perfect the emergency medical service system.
